摘要
目的 探讨中医药治疗对HIV/AIDS患者接受抗病毒治疗后CD4^(+)T淋巴细胞计数长期变化趋势的影响。方法 以“艾滋病综合防治数据信息系统”和“中医药治疗艾滋病数据库”为数据来源,回顾性收集2004年参与河南中医项目地区的HIV/AIDS患者的基线信息及随访年的CD4^(+)T淋巴细胞计数。按是否参与中医项目分为中医治疗组和非中医治疗组,利用倾向性评分匹配法来控制组间差异,并观察两组患者CD4^(+)T淋巴细胞计数的变化趋势。结果 本研究共纳入2 046例HIV/AIDS患者,其中中医治疗组973例,非中医治疗组1 073例,匹配后共得到723对病例,组间差异无统计学意义(P>0.05)。两组CD4^(+)T淋巴细胞计数年变化趋势图显示,CD4^(+)T淋巴细胞计数<200个/μL和200~350个/μL的HIV/AIDS患者在治疗初期CD4^(+)T淋巴细胞计数显著升高,中医治疗组升高得更快。当CD4^(+)T淋巴细胞计数达到350个/μL左右时,组间差异消失,升幅减缓;CD4^(+)T淋巴细胞计数在350~500个/μL的HIV/AIDS患者,治疗后CD4^(+)T淋巴细胞计数略有升高,组间无差异;CD4^(+)T淋巴细胞计数>500个/μL的HIV/AIDS患者的CD4^(+)T淋巴细胞计数在短暂下降后维持在特定水平。结论 中医药对HIV/AIDS患者CD4^(+)T淋巴细胞计数的影响与基线CD4^(+)T淋巴细胞计数水平有关,能显著提高CD4^(+)T淋巴细胞计数<350个/μL的HIV/AIDS患者的CD4^(+)T淋巴细胞计数。
Objective To study the effect of TCM therapy on the long-term trend of CD4^(+)T cell counts among HIV/AIDS patients.Methods By using the database of TCM Treatment for AIDS and AIDS prevention and control information system as the data source, we collected the baseline information and annual CD4^(+)T cell counts of HIV/AIDS patients who participated in the Nation Free TCM HIV/AIDS Treatment program in 2004 in Henan province of China retrospectively. HIV/AIDS patients were divided into TCM+cART group and cART group by whether taking TCM. We used propensity score matching to control the differences between groups, and observed the figure of annual CD4^(+)T cell counts.Results A total of 2 046 HIV/AIDS patients were enrolled in this study, 973 in TCM+cART group and 1 073 in cART group. Propensity score matching yielded 723 matched pairs, there was no statistically significant difference between the two groups(P>0.05). The annual CD4^(+)T cell counts trends after PSM shown that, for HIV/AIDS patients with CD4^(+)T cell counts<200 cells/μL and of 200-350 cells/μL, the trend of CD4^(+)T cell counts in TCM+cART group increased rapidly, among the two groups, TCM+cART group enjoyed a faster growth. After the baseline CD4^(+)T cell counts reaches about 350 cells/μL, the difference between the group disappears, and the increase slow down. For HIV/AIDS patients with baseline CD4^(+)T cell counts of 350-500 cells/μL, the CD4^(+)T cell counts increase slightly, but there was no difference between the two groups. For HIV/AIDS patients with baseline CD4^(+)T cell counts of ≥500 cells/mL, the CD4^(+)T cell counts tended to be maintained at a particular level after a brief dip.Conclusion The effect of TCM therapy on CD4^(+)T cell counts of HIV/AIDS patients is related to their baseline CD4^(+)T cell counts level. TCM therapy can increase CD4^(+)T cell counts among HIV/AIDS patients with CD4^(+)T cell counts<350 cells/μL.
