摘要
目的了解天津市青少年二手烟暴露现状,探讨其影响因素,为制定相关控烟政策提供依据。方法用多阶段分层整群随机抽样方法,在天津市抽取10个区的30所初中、20所普高和10所职业学校,共7568名在校学生自填纸质问卷匿名参与调查。采用复杂抽样加权分析方法,用SPSS 25.0软件复杂抽样分析程序进行χ^(2)检验和多因素Logistic回归分析。结果64.1%的学生报告自己在过去7 d内暴露于二手烟,男生(66.5%)高于女生(61.5%),普高学生(69.5%)高于初中学生(62.4%)、职业学校学生(59.6%),差异均有统计学意义(P<0.05或P<0.001)。学生在不同场所暴露于二手烟的比例由高到低分别为室外公共场所(51.5%)、室内公共场所(47.0%)、家(33.0%)和公共交通工具(23.6%)。多因素Logistic回归分析显示,与职业学校学生相比,普高学生(OR=1.828)和初中学生(OR=1.726)二手烟暴露风险更高;父母吸烟(OR=2.805)、朋友吸烟(OR=2.690)、看到过老师吸烟(OR=1.576)、每周可支配金额>50元(OR=1.230)的学生暴露于二手烟的可能性更大。结论天津市青少年二手烟暴露情况较严重,建议多部门协作,积极推进室内公共场所全面禁烟,倡导室外公共场所控烟,加强无烟家庭建设,落实学校、公共交通工具100%无烟。
Objective To understand the current situation of secondhand smoking exposure and its influencing factors among adolescents in Tianjin,and provide a basis for formulating relevant tobacco control policy.Methods By using multi-stage stratified cluster random sampling method,30 junior high schools,20 high schools,and 10 vocational schools in 10 districts of Tianjin were selected.A total of 7568 students participated in the survey and filled out paper questionnaires anonymously.Data analysis used complex sampling weighted analysis method,and SPSS 25.0 software complex sampling analysis program was used forχ^(2)test and multivariate logistic regression analysis.Results 64.1%of the students reported that they had been exposed to secondhand smoking in the past 7 days.The reported percentage in male students(66.5%)were higher than that of female students(61.5%),high school students(69.5%)higher than junior high school students(62.4%)and vocational school students(59.6%),and the differences were statistically significant(P<0.05 or P<0.001).The proportion of students exposed to secondhand smoke in different places from high to low were outdoor public places(51.5%),indoor public places(47.0%),home(33.0%)and public transportation(23.6%).The multivariate logistic regression analysis showed that high school students(OR=1.828)and junior high school students(OR=1.726)were more likely to suffer from the secondhand smoke exposure compared with vocational school students.The students who had at least one of parents was smoker(OR=2.805),or at least one of friends was smoker(OR=2.690),or had seen at least one of teachers smoking(OR=1.576),and the weekly disposable money>50 yuan(OR=1.230)had more risk on secondhand smoking exposure.Conclusion The situation of secondhand smoking exposure among adolescents in Tianjin is serious.It is recommended that multi-departmental cooperation should be actively promoted to completely ban smoking in indoor public places,advocate tobacco control in outdoor public places,strengthen the construction of smoke-free families,and implement 100%smoke-free schools and public transportation.
