正畸治疗对牙龈沟及口腔微生态影响的研究 被引量:4

The impact of orthodontic treatment on the microecology of gingival sulcus and oral cavity

摘要 目的探讨口腔正畸治疗对牙龈沟及口腔细菌微生态的影响。方法选取2016年7月~2017年6月期间的正畸治疗患者56例作为观察组,同期56例口腔健康人群作为对照组,分别对观察组正畸治疗前、治疗1个月后及对照组进行牙龈沟、口腔细菌、碱性磷酸酶(ALP)活性检测,并对比分析。结果正畸治疗1个月后,观察组牙龈沟球菌检出率(32.35±12.02)%,明显低于对照组(53.87±11.74)%,牙龈沟核酸杆菌39.29%、卟啉单胞菌19.64%、伴放线放线杆菌44.64%、福赛类杆菌26.79%检出率明显高于对照组,差异均有统计学意义(P<0.05);正畸治疗1个月后,观察组ALP活性明显增高,(151.42±41.24)uU/30 s,高于对照组(95.01±40.83)uU/30 s,差异有统计学意义(P<0.05)。结论口腔正畸治疗可在很大程度上对牙龈沟及口腔的细菌微生态产生破坏作用,引发多种口腔疾病。因此,在接受口腔正畸治疗时,应注意口腔清洁,从而减少由此引发的牙齿损害,提高正畸治疗效果。 Objective To discuss the impact of orthodontic treatment on the microecology of gingival sulcus and oral cavity.Methods 56 patients who received orthodontic treatment during July 2016 and June 2017 in our department were enrolled into the observation group,meanwhile,56 healthy individuals were recruited into the control group.Samples were collected from gingival sulcus and oral cavity before treatment and one month after treatment for bacteria culture.ALP activity was also determined.Results One month after orthodontic treatment,the detection rate of cocci in gingival sulcus was significantly lower in the observation group than that in the control group(32.35±12.02)%vs(53.87±11.74)%;P<0.05.The detection rates of gingival sulcus nucleo-bacillus 39.29%,Porphyromonas 19.64%,Actinobacillus actinomycetes 44.64%and Forsythia 26.79%increased significantly one month after the treatment in the observation group,and were significantly higher in the observation group than those in the control group(P<0.05).One month after orthodontic treatment in the observation group,the activity of ALP(151.42±41.24 uU/30 s)increased,and was significantly higher than that in the control group(95.01±40.83)uU/30 s(P<0.05).Conclusions Orthodontic treatment can affect the microecology of gingival sulcus and oral cavity,which are accountable for various oral diseases.Therefore,during orthodontic treatment,patients should maintain oral cleanliness in order to minimize the damage of teeth.
