摘要
目的分析贵州医科大学附属医院就诊的女性人乳头瘤病毒(human papilloma virus,HPV)感染情况及亚型分布特点。方法收集2020年1—12月贵州医科大学附属医院行宫颈脱落细胞检查的宫颈拭子样本8427例,采用聚合酶链式反应(polymerase chain reaction,PCR)技术进行基因分型检测,分析其分布特征和感染特点。结果贵阳市女性HPV总感染率为28.23%(2379/8427)。在不同年龄组中,≥60岁组与0~<20岁组感染率较高,分别为43.57%(173/397)和41.67%(20/48)。感染基因型分布依次为HPV52、HPV16、HPV58、HPV53和HPV39,分别为619例(18.41%)、401例(11.92%)、362例(10.76%)、236例(7.02%)、212例(6.30%)。高危型HPV感染2046例(24.28%),其次是中危型385例(4.57%),低危型感染325例(3.86%)。HPV感染以单一基因型感染为主,占73.52%(1749/2379);二重感染发生率为18.28%(435/2379),其中HPV52+HPV16两种亚型较为常见。结论贵州医科大学附属医院就诊的女性HPV感染率较高,且以单一高危型感染为主,为针对性采取防控策略,应加大HPV筛查及宣传HPV感染途径和危害,增加HPV疫苗数量,可有效减少感染率和降低子宫颈癌发生风险。
Objective To analyze the prevalence of human papilloma virus(HPV)infection and characteristics of subtype distribution among women treated at the Affiliated Hospital of Guizhou Medical University.Methods 8427 cervical swab samples were collected from the Affiliated Hospital of Guizhou Medical University from January to December 2020.HPV genotypes were detected by quantitative fluorescence polymerase chain reaction(PCR).The distribution and characteristics of HPV infection were analyzed.Results The overall prevalence of HPV infections was 28.23%(2379/8427)among the included women.The highest HPV infection rates were found in the≥60 and 0-<20 years age groups,which were 43.57%(173/397)and 41.67%(20/48),respectively.The main genotypes were HPV52,HPV16,HPV58,HPV53,and HPV39,accounting for 18.41%(619 cases),11.92%(401 cases),10.76%(362 cases),7.02%(236 cases),6.30%(212 cases)of the infections,respectively.The number of infection by high-risk HPV genotype was 2046(24.28%),followed by medium-risk HPV infection 385(4.57%),and the number of infection due to low-risk HPV was 325(3.86%).The infection due to single genotype was the most common,accounting for 73.52%(1749/2379)of all the infected cases.The proportion of co-infection was 18.28%(435/2379),mostly being HPV52 and HPV16 co-infection.Conclusions The HPV infection is prevalent among women visitingi the Affiliated Hospital of Guizhou Medical University,primarily being single infection by one high-risk genotype.To adopt targeted prevention and control strategies,HPV screening and education about HPV transmission and hazards should be strengthened,and the uptake of HPV vaccines be promoted,which may effectively reduce the HPV infection rate and the risk of cervical cancer.
