摘要
目的研究分析彩超诊断急性坏疽胆囊炎合并胆囊穿孔的临床声像图特征。方法选择我院于2017年2月至2019年2月收治的28例急性坏疽胆囊炎合并胆囊穿孔患者为本次回顾性研究对象,对其应用彩色多普勒超声检查,观察其检出符合情况与声像特征。结果彩超显示胆囊肿大15例、胆囊正常大小8例、胆囊轮廓不清5例,胆囊处于极高张力状态且无回声暗区消失,胆囊区域存在形态不规则及低回声的不均质团块,而团块中伴有强回声并后方伴随声影;22例患者存在胆囊壁毛糙粗厚及局部膨出或缺损征象,胆囊周围有局限性积液暗区。结论对急性坏疽胆囊炎合并胆囊穿孔应用彩色多普勒超声诊断,检出符合率较高,用于早期诊断有着重要参考价值。
Objective To study and analyze the clinical ultrasonographic features of acute gangrenous cholecystitis with gallbladder perforation diagnosed by color doppler ultrasonography.Methods Twenty-eight patients with acute gangrenous cholecystitis complicated with gallbladder perforation were selected from February 2017 to February 2019 in our hospital as the subjects of this retrospective study,and were examined by color doppler ultrasonography to observe the coincidence and sonographic characteristics.Results Color doppler ultrasound showed gallbladder enlargement in 15 cases,normal gallbladder size in 8 cases,blurred gallbladder contour in 5 cases,the gallbladder is in a state of high tension and the anechoic dark area disappears.There are irregular and hypoechoic heterogeneous masses in the gallbladder area,while the masses are accompanied by strong echo and sound shadow in the back.And the masses were accompanied by strong echo occlusion in the rear of the gallbladder wall hair in 22 cases.Rough and thick,with signs of local swelling or defect,and a localized dark area of hydrocele around the gallbladder.Conclusion The diagnostic accuracy of color doppler ultrasonography in acute gangrenous cholecystitis with gallbladder perforation is high,and it has important reference value for early diagnosis.
