摘要
目的探讨椎体成形术应用于骨质疏松椎体压缩骨折患者术后相邻椎体再发骨折的危险因素。方法筛选200例2015年4月至2018年5月本院收治的200例骨质疏松椎体压缩骨折患者作为研究对象,给予所有患者经皮椎体成形术,明确患者治疗效果和预后情况。结果分析相邻椎体骨折组与非相邻椎体骨折组住院中的基本资料,性别、年龄、骨折椎体个数、经皮椎体成形术手术椎体数、骨水泥用量及身高下降数据,无统计学意义,P>0.05;体重指数对比,差异呈统计学意义,P<0.05;单因素分析性别、年龄、骨折椎体个数、体重指数、既往骨折史、糖皮质激素使用史、胃肠道疾病、糖尿病、高血压、肺气肿对再发骨折的影响,提示体重指数≤18.5 kg/m^2为主要危险因素,P<0.05。结论椎体压缩后楔形变,导致改变脊柱生理曲度,强化伤椎骨水泥后存在着过大的强度,特别是发生灌注剂渗漏时,会降低椎间盘功能,伤椎对相邻椎体产生的不良反应,会增加相邻椎体发生骨折的概率。
Objective To explore the risk factors of the treatment of the refracture of the adjacent vertebral bodies after the operation of the vertebroplasty in the patients with osteoporotic vertebral compression fracture.Methods 200 patients with osteoporotic vertebral compression fracture treated by our hospital from April 2015 to May 2018 were selected as the subject of the study,and all patients were given percutaneous vertebroplasty,and the treatment effect and the prognosis of the patients were determined.Results The basic data,sex,age,the number of vertebral bodies,the number of vertebral bodies,the amount of bone cement and the height of the reduction of the height of the adjacent vertebral fracture group and the non-adjacent vertebral fracture group were analyzed,and there was no statistical significance,P>0.05,and the body weight index was compared.The difference was statistically significant,P<0.05;single factor analysis of gender,age,number of vertebral bodies,body mass index,history of previous fractures,history of use of corticosteroids,gastrointestinal disorders,diabetes,hypertension,and emphysema on reoccurring fractures,It was suggested that the body weight index was 18.5 kg/m2 as the main risk factor,P<0.05.Conclusion The wedge-shaped change after the compression of the vertebral body leads to the change of the physiological curvature of the spinal column,and the excessive strength exists after the vertebral cement is strengthened,in particular,when the perfusion agent is leaked,the function of the intervertebral disc is reduced,and the adverse reaction generated by the vertebra on the adjacent vertebral bodies can be reduced,and the probability of fracture of the adjacent vertebral bodies can be increased.
