摘要
目的探讨罗哌卡因腰-硬联合阻滞麻醉在人工髋关节置换术中的应用及对患者并发症发生的影响。方法选取2017年5月-2020年5月我院收治的82例接受人工髋关节置换术患者,随机分为两组,对照组进行罗哌卡因硬膜外麻醉,研究组进行罗哌卡因腰-硬联合阻滞麻醉。比较两组不良反应发生率、治疗效果、临床指标。结果与对照组比较,研究组不良反应发生率更低(P<0.05);与对照组比较,研究组心率更低,平均动脉压更高,咪芬合剂使用次数、局部麻醉诱导量更少,麻醉起效时间更短(P<0.05);与对照组比较,研究组Bromage评分、感觉阻滞最高平面更高,最大运动及感觉阻滞时间、恢复时间更短(P<0.05)。结论在人工髋关节置换术中,罗哌卡因腰-硬联合阻滞麻醉可维持正常心率、平均动脉压,提高Bromage评分,缩短运动及感觉阻滞时间,是一种安全且有效的麻醉方式。
Objective To investigate the application of ropivacaine combined spinal epidural anesthesia in total hip arthroplasty and its influence on complications.Methods 82 patients with total hip arthroplasty in our hospital from May 2017 to May 2020 were selected and randomly divided into two groups.The control group was given ropivacaine epidural anesthesia,and the study group was given ropivacaine combined spinal epidural anesthesia.The incidence of adverse reactions,treatment effect,clinical indicators were compared between the two groups.Results Compared with the control group,the incidence of adverse reactions in the study group was lower(P<0.05);compared with the control group,the heart rate in the study group was lower,the average arterial pressure was higher,the use times of mifene mixture,the induction amount of local anesthesia were less,and the onset time of anesthesia was shorter(P<0.05);compared with the control group,the Bromage score,the highest level of sensory block,the maximum motor and sensory block in the study group were higher Time and recovery time were shorter(P<0.05).Conclusion In total hip arthroplasty,ropivacaine combined spinal epidural anesthesia can maintain normal heart rate,mean arterial pressure,improve Bromage score,shorten the time of motor and sensory block,which is a safe and effective anesthesia method.
