摘要
目的观察加味半夏泻心汤联合百令胶囊治疗糖尿病肾病的疗效。方法将2016年11月-2018年4月庆阳市第二人民医院内分泌肾病科收治的60例糖尿病肾病属气阴两虚型兼湿浊中阻者随机分为观察组和对照组,每组各30例,2组均接受糖尿病肾病常规治疗,观察组加服加味半夏泻心汤联合百令胶囊治疗,2组疗程均为6月。观察2组患者的临床疗效与治疗前后肾功能变化、24h尿蛋白定量、全段甲状旁腺激素(iPth)及肾小球滤过率。结果治疗后,观察组总有效率93.33%,对照组总有效率为63.33%,差异有统计学意义(P<0.01)。2组患者肾功较治疗前均降低(P<0.05),治疗前后观察组较对照组肾功改善明显,差异有统计学意义(P<0.05)。治疗后,2组患者治疗后24h尿蛋白定量、全段甲状旁腺激素(iPth)及肾小球滤过率均有显著改善,治疗后观察组24h尿蛋白定量下降幅度明显优于对照组,差异均有统计学意义(P<0.05)。结论采用加味半夏泻心汤联合百令胶囊治疗气阴两虚兼湿浊中阻型糖尿病肾病患者,可明显提高疗效,改善肾功能、24h尿蛋白定量、全段甲状旁腺激素(iPth)及肾小球滤过率均有显著改善,疗效优于单纯西药治疗。
Objective To observe curative effect of modified pinellia heart-draining decoction combined with bailing capsule on diabetic nephropathy.Methods The paper chose 60 cases diabetic nephropathy patients with deficiency of Qi and Yin type,turbi damp obstructing in middle-jiao in endocrine nephropathy department of Qingyang Cith The Second People’s Hospital from November 2016 to April 2018,and divided them into observation group and control group,with 30 cases in each group.Control group was treated with routine treatment of diabetic nephropathy,and observation group with modified pinellia heart-draining decoction combined with bailing capsule,with treatment course of 6 months.Clinical effect,changes of renal function,24-hour urinary protein,whole segment parathyroid hormone(iPTH)and glomerular filtration rate were observed before and after treatment.Results After treatment,total effective rate was 93.33%in observation group and 63.33%in control group,with significant difference between two groups(P<0.01).Renal function between two groups was lower than before treatment(P<0.05).Renal function of observation group was significantly better than control group before and after treatment,(P<0.05).After treatment,24-hour urinary protein quantity,whole parathyroid hormone(iPTH)and glomerular filtration rate between two groups was significantly improved,24-hour urinary protein quantity of observation group was significantly better than control group,difference was statistically significant,(P<0.05).Conclusion Modified pinellia heart-draining decoction combined with bailing capsule can improve curative effect,renal function,24h urinary protein,total parathyroid hormone(iPth)and glomerular filtration rate(PFR)of patients with type 2 diabetic nephropathy with both Qi Yin deficiency and dampness turbidity significantly,with better curative effect than Western medicine only.
