摘要
目的研究小剂量多巴胺联合多巴酚丁胺对重症肺炎患儿肺功能及IL-8、CRP水平的影响。方法回顾性选取112例患儿作为研究对象,疾病类型:重症肺炎,选取时间:2019年6月-2020年6月,分组:对照组(56例)和观察组(56例),分组依据:治疗方式。对照组使用多巴酚丁胺治疗,试验组联合小剂量多巴胺治疗,两组均治疗7 d。对两组治疗后临床疗效,治疗前后肺功能、症状消失时间及血清IL-8、CRP水平比较。结果治疗后,试验组临床总有效率(89.29%),相比对照组(66.07%),明显处于较高水平;治疗后,两组PEF、TEF50%与治疗前相比,均呈现出明显降低趋势,且试验组较对照组均低;两组TPEF/TE均呈现出明显升高的趋势,且试验组较对照组高;两组治疗后血清IL-8、CRP水平与治疗前相比均降低,且较对照组,试验组更低;治疗后,相较于对照组,试验组患者体温消退、呼吸困难缓解、心率恢复正常、啰音消失各项时间均相对更短(均P<0.05)。结论重症肺炎患儿应用小剂量多巴胺联合多巴酚丁胺治疗,可有效缓解临床症状,改善肺功能,抑制炎症反应,缩短患者的临床症状消失时间。
Objective To study effect of low-dose dopamine combined with dobutamine on pulmonary function and levels of IL-8 and CRP of children with severe pneumonia.Methods The paper reviewed and chose 112 children with severe pneumonia from June 2019 to June 2020 as study objects,and divided them into control group(56 cases)and observation group(56 cases).Grouping basis:treatment method.Control group was treated with dobutamine,and experimental group with low-dose dopamine.Both groups were treated for 7 days.Clinical effect,pulmonary function,symptom disappearance time and serum IL-8 and CRP levels were compared before and after treatment between two groups.Results After treatment,total clinical effective rate of experimental group(89.29%)was significantly higher than control group(66.07%).After treatment,PEF and TEF50%in two groups showed significant downward trend compared with before treatment,that of experimental group was lower than control group;TPEF/TE in both groups showed a significant upward trend,that of experimental group was lower than control group.Levels of serum IL-8 and CRP in two groups after treatment were lower than before treatment,those of experimental group were lower than control group.After treatment,temperature to subside,dyspnea to ease,heart rate to normal and rale to disappear in experimental group was relatively shorter than control group(all P<0.05).Conclusion Low dose dopamine combined with dobutamine can effectively alleviate clinical symptoms in treatment of children with severe pneumonia,improve pulmonary function,inhibit inflammatory reaction,and shorten disappearance time of clinical symptoms.
