预防牙齿脱矿和促进再矿化药物的研究进展 被引量:6

摘要 龋病是发病率较高的口腔慢性感染性疾病,防龋的重点之一是抑制早期龋脱矿和促进再矿化。近年来,在口腔疾病的防治工作中大量应用氟化物、生物矿化物质和天然药物等,通过影响脱矿和再矿化的动态平衡过程来防龋。氟化物作为最早应用于临床的局部防龋药物具有疗效佳、操作简便和价格实惠的优势但又因其毒副作用限制了它的推广和使用;生物矿化物质是具有优良的生物相容性、有效性和安全性的新兴材料,通过改善牙齿表面形态、改变孔隙结构、蛋白改性来影响脱矿和再矿化进程;天然药物的使用由来已久,它富含多种天然生物活性因子及有效成分,作用机制不同于氟化物,可作用于龋损深部减缓有机和无机基质的损失从而实现抑制脱矿和促进再矿化,疗效持久。本文将总结概括现有的预防牙齿脱矿和促进再矿化药物的相关研究进展,其中包括对上述三种类型药物的特点及对它们的改性、联合、创新等相互取长补短的方式的研究进行系统的回顾和概述,以期为基础研究和临床应用提供参考。 Dental caries is the most common chronic infectious oral disease.One of the key points of caries prevention is to inhibit early caries demineralization and promote remineralization.In recent years,fluoride,biomineral substances and natural drugs have been widely used in the prevention and treatment of oral diseases by affecting the dynamic equilibrium process of demineralization and remineralization.As the earliest clinical anti-caries drug,fluoride has the advantages of good curative effect,simple operation and affordable price,but it is limited by promotion and use due to its toxic side effects.Biomineralized materials are new materials with excellent biocompatibility and safety.The process of demineralization and re-mineralization can be affected by improving the surface morphology of teeth by modifying pore structure proteins.Natural medicines have a long history of use.They are rich in a variety of natural biologically active factors and active ingredients.The mechanism of action is different from that of fluorides.It can slow down the loss of organic and inorganic matrix in deep caries lesions,thus inhibiting demineralization and promoting remineralization,with a lasting effect.This paper will summarize the current research progress in the prevention of demineralization of teeth and the promotion of remineralization drugs,including the study of the characteristics of the above three types of drugs,such as modification,association,and innovation and complement eachother’s strengths,in order to provide reference for basic research and clinical application.
