热休克蛋白47在胆道闭锁患儿肝组织中的表达

摘要 目的 研究患儿胆道闭锁(BA)儿童肝组织中热休克蛋白47(HSP47) 与肝脏纤维化、手术预后的关系.方法 选取深圳市儿童医院小儿肝胆外科行葛西手术的38例胆道闭锁患儿,分别根据肝纤维化Ohkuma's病理分级、手术时年龄分组,采用SP免疫组化法测定肝脏组织中HSP47的表达,结合术后随访资料进行统计学分析.结果 38例均获随访,自体肝生存29例,占76.32%;HSP47在肝纤维化程度较重组的表达明显高于较轻组(Z=5.196,P<0.01);随着手术时年龄的增加HSP47表达增强(r=0.598,P<0.01),HSP47表达强度越高,术后胆管炎发作越多(r=0.361,P<0.05);HSP47表达强度较高者其自体肝生存率较低(P<0.05).结论 胆道闭锁患儿肝脏组织HSP47的表达与肝组织纤维化程度密切相关,HSP47表达强度可能为胆道闭锁患儿预后的影响因素之一,可作为评价葛西术后疗效的参考指标.
