基于两性霉素B和氟康唑治疗基础上的真菌性角膜炎发病机制探究

摘要 目的:在两性霉素B和氟康唑治疗基础上探究真菌性角膜炎发病机制探究。方法:分析比较2021年12月至2023年12月我院收治的54例(54只眼)真菌性角膜炎患者经两性霉素B联合氟康唑交替滴眼治疗前及治疗后2 d、4 d、15 d、30 d患者血清中IL-1、IL-6、TNF-α、Dectin-1、Nrf2、HO-1的表达水平。结果:与用药前比较,用药后2 d,IL-1、IL-6、TNF-α、Dectin-1血清水平下降,Nrf2、HO-1水平上升,但差异未达到统计学意义,用药后4 d开始,血清IL-1、IL-6、TNF-α、Dectin-1水平下降,及Nrf2、HO-1上升水平均较用药前有统计学意义。结论:角膜感染真菌后,Dectin-1促进角膜病变进展,Nrf2、HO-1促进角膜恢复。 Objective: To explore the pathogenesis of fungal keratitis on the basis of amphotericin B and fluconazole. Methods: The expression levels of IL-1, IL-6, TNF-α, Dectin-1, NRF2 and HO-1 in serum of 54 patients (54 eyes) with fungal keratitis admitted to our hospital from December 2021 to December 2023 were analyzed and compared before and after alternating eye drops of amphotericin B combined with fluconazole for 2 d, 4 d, 15 d and 30 d. Results: Compared with before medication, the serum levels of IL-1, IL-6, TNF-α and Dectin-1 decreased 2 days after medication, while the levels of Nrf2 and HO-1 increased, but the difference was not statistically significant. From 4 days after medication, the serum levels of IL-1, IL-6, TNF-α and Dectin-1 decreased, and also the levels of Nrf2 and HO-1 increased, all of which were statistically significant compared with before medication. Conclusion: Dectin-1 promotes the progression of keratopathy and NRF2 and HO-1 promote corneal recovery after corneal fungal infection.
