摘要
目的 探讨经阴道彩色多普勒超声 (TVCDS)对子宫腺肌病与子宫肌瘤的鉴别诊断价值。方法 回顾性总结分析了经手术和病理证实的 2 6例子宫腺肌病和 4 3例子宫肌瘤的声像图特征 ,并将两者进行比较分析。结果 子宫腺肌病表现子宫增大无形态改变 ,病灶多位于后壁 (占 80 % ) ,病灶区呈现粗颗粒状稍强回声 ,间以大小不等的低回声区 ,病灶区见稀疏星点状血流 ,频谱显示阻力指数较高 ,RI0 .73± 0 .0 8。子宫肌瘤可发生在子宫任何部位 ,肌瘤边界清楚 ,内部出现特有的漩涡样回声 ,肌瘤周围有环绕血流 ,频谱显示阻力指数偏低 ,RI0 .5 9± 0 .0 5。结论 TVCDS能很好的显示子宫腺肌病和子宫肌瘤的声像图特征 。
Objective To explore the transvaginal Doppler sonographic(TVCDS)value in differential diagnosis of endometrioma and hysteromyoma.Methods We Sumerized retrospectively the sonograms of 26 patients with endometrioma and 43 patients with hysteromyoma comfirmed by operation and pathology and comparatively analysed the characteristics of the two diseases.Results The appearance of hysteromyoma was an enlarged uterus without morphological changes.The focus area which located at the posterior wall of the uterus(80%)revealed coarse granular strong echoes with intermittent aniso low echoes and with scattered esteroid blood flow the Doppler of which had high RI,0 73±0 08.Hysteromyoma occurred on any area of the uterus with clear boundary and with internal special turbulant blood flow.At the circumference of the tumor there was encircled blood flow the Doppler of which had low resistence index,RI=0 59±0 05.Conclusions TVCDS can display excellently the characteristics of endometrioma and hyteromyoma,beneficial for differential diagnosis.
