摘要
目的 :探讨基质金属蛋白酶 9(MMP 9)在围体外循环术 (CPB)中的变化及其与肺损伤的相关性。 方法 :临床选择 2 0例接受CPB术的患者 ,分别测定CPB开始 (术中进行肝素化之前 )、开始后 5min、结束时 (鱼精蛋白中和之前 )、结束后 1h和 6h血浆MMP 9浓度 ,同时测定CPB结束后各时间点的肺泡 动脉氧分压差 (A aDO2 )、呼吸指数 (RI) ,记录主动脉阻断时间、转流时间及拔管时间。 结果 :①在CPB结束时及结束后 1h血浆MMP 9的浓度与基础值及CPB开始后 5min相比有显著差异 (P <0 .0 1) ,其中在CPB结束时血浆MMP 9的浓度达到最高峰值 ,之后呈下降趋势 ,6h后接近基础值 ;②血浆MMP 9浓度与主动脉阻断时间有较明显的相关性 ,与转流时间相关性不甚明显 ;③由于术后肺损伤实际发生率较低 ,尤其是早期肺损伤不易测定 ,因此在血浆MMP 9浓度的变化与术后肺功能损伤之间未见有明显相关性。 结论 :CPB可以引起血浆MMP 9浓度的明显增高 。
Objectives: To study the correlation between plasma matrix metalloproteinase 9(MMP 9) concentration and acute lung injury following cardiopulmonary bypass(CPB). Methods: Human plasma was obtained after informed consent from twenty patients undergoing CPB. Plasma was collected at the beginning of CPB, 5 minutes after the initiation of CPB, at the termination of CPB, 1 hour after the termination of CPB and 6 hours after the termination of CPB. All samples were analyzed by standard enzyme linked immunosorbent assay (ELISA). A aDO 2 and respiratory index (RI) was measured at the termination, 1 hour and 6 hours after termination of CPB. The cross clamp times, CPB times and the time to extubation was recorded. Data were expressed as means ±SE and assessed by analysis of variance (ANOVA).The regression analysis was utilized to define the correlations of variables measured( A aDO 2 ,RI, cross clamp times, CPB times and the time to extubation ) at the end of CPB. Results: Plasma MMP 9 concentration was significantly increased at the end of CPB (430.6±50)μg/L( P < 0.01 ) as compared with both the start of CPB(42.6±3.2)μg/L and 5 minutes after the initiation of CPB (97.5±22.8)μg/L. 6 hours after the termination of CPB plasma MMP 9 concentration was close to the concentration of basement level. The correlation between plasma MMP 9 concentration at the end of CPB and cross clamp times was significant. The correlation between lung injury and increased plasma MMP 9 concentration was not significant as we were unable to demonstrate any pulmonary dysfunction in these 20 patients. Conclusions: Patients undergoing CPB showed a significant increase in plasma MMP 9 levels and the effect of cross clamp time was more remarkable.
