摘要
目的通过检测多发性骨髓瘤(MM)患者外周血淋巴/单核细胞比值(LMR)、淋巴细胞亚群水平,评估MM患者免疫功能状态及与预后的关系。方法选取2015年1月至2017年6月医院收治的57例MM患者作为病例组,并选取同时期健康对照30例作为对照组,收集和分析研究对象外周血淋巴、单核细胞数,T、B、NK以及CD4/CD8双阳性T淋巴细胞(DP)、CD4/8双阴性T淋巴细胞(DN)水平,计算LMR并分析其与MM患者血清β2微球蛋白(β-2-MG)、白蛋白(ALB)的相关性。结果与对照组相比,MM组LMR、T淋巴细胞、Ts细胞比例增高且差异具有统计学意义,而Th/Ts、B淋巴细胞比例均减低,虽然NK淋巴细胞比例增高,但差异无统计学意义。T淋巴细胞、Ts细胞与白蛋白呈正相关,NK淋巴细胞与白蛋白呈负相关,CD4/8双阴性细胞与β2-微球蛋白呈负相关。结论 LMR与淋巴亚群水平异常、多发性骨髓瘤患者疾病进展及预后相关。
Objective To evaluate immune state and analyze its prognostic function of patients with multiple myeloma(MM)through detect the changes of absolute lymphocyte count/absolute monocyte count(LMR)and lymphocyte subsets[T,Th,Ts,CD4(+)CD8(+)T cells,CD4(-)CD8(-)T cells,B,NK]in peripheral blood.Methods A total of57 MM patients in our hospital from January 2015 to January 2017 were enrolled and 30 healthy persons were enrolled as control group.The lymphocyte counts and monocyte counts were detected for the LMR.Meanwhile the levels of lymphocyte subsets(T,Th,Ts,DP,DN,B,NK)were measured by flow cytometer.And differences of detected indicators between the two groups were compared.Correlations between LMR,lymphocyte subsets andβ2-MG,serum albumin were analyzed.Results The levels of LMR,total T lymphocyte and Ts lymphocyte in MM group were higher than the normal control with statistic significance.And the ratio of Th/Ts and B lymphocyte were decreased compare with normal control.The levels of T lymphocyte and Ts lymphocyte had positive correlations with serum albumin,while NK lymphocyte had negative correlation with serum albumin.The level of DN lymphocyte had negative correlation withβ2-MG.Conclusion The abnomal level of LMR and lymphocyte subsets are related with progression and prognosis of MM patients.
