摘要
目的应用二维斑点追踪成像(2D-STI)技术探讨妊娠期高血压疾病(HDCP)患者左心室收缩期纵向、径向及圆周应变的特点,评价HDCP患者左心收缩功能。方法 HDCP患者34例(HDCP组),正常妊娠妇女30例(对照组)。经胸超声采集并储存左心室短轴二尖瓣水平、乳头肌水平及心尖水平,左心室长轴心尖四腔、三腔及两腔切面的二维图像,运用QLAB分析软件脱机分析。结果 (1)HDCP组左心室心肌各节段收缩期纵向应变、径向应变及圆周应变值较对照组明显减低,差异有统计学意义(P<0.01);(2)两组左心室长轴心肌各节段收缩期纵向应变绝对值呈现自基底段至心尖段逐渐增加趋势。结论 HDCP患者左心室射血分数正常时,2D-STI技术可以发现左心室心肌收缩功能受损,为临床评价其心功能提供了又一有效方法。
Objective To assess left ventricular systolic function with the peak systolic strain of longitudinal, radial and circumferential in patients with essential hypertension by using speckle tracking imaging. Methods Study population consisted of 34 patients with hypertensive disorder complicating pregnancy and 30 healthy subjects. The images were recorded from the left ventricular short-axis views at the levels of mitral annulus, papillary muscle and apex, and the apical four-chamber view, two-chamber view and long-axis view of the left ventricle respectively. Then these images were analyzed by off-line 2D-STI analysis Qlab software. Results(1)The value of longitudinal strain, radial strain and circumferential strain in HDCP group were lower than those of case group(P<0.05 or P<0.01);(2)The value of longitudinal strain were increased from the base to the apex no matter HDCP group or control group(P<0.05 or P<0.01). Conclusions When the EF is normal in patients with hypertensive disorder complicating pregnancy, 2D-STI can found the myocardial systolic function have been damaged, providing another useful modality for evaluating cardiac function.
