摘要
目的探讨血清脂代谢指标联合血清CEA、CA72-4、CA19-9检测诊断结直肠癌的临床意义。方法以结直肠癌患者100例(肿瘤组)、结直肠息肉患者100例(息肉组)及100例健康体检者(对照组)为研究对象,使用生化分析方法检测所有受试者血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)等脂代谢指标及血清癌胚抗原(CEA)、癌抗原72-4(CA72-4)及糖链抗原19-9(CA19-9)等肿瘤标志物水平。结果肿瘤组患者血清TC、TG、HDL-C和LDL-C水平均明显低于息肉组和对照组,差异具有统计学意义(P<0.05)。肿瘤组患者血清CEA、CA72-4、CA19-9水平均明显高于息肉组和对照组,差异具有统计学意义(P<0.05)。肿瘤组患者血清TC、LDL-C水平分别与CEA、CA72-4、CA19-9有相关性,但TG、HDL-C水平与CEA、CA72-4、CA19-9无明显的相关性。结论 CEA、CA72-4、CA19-9联合血清脂代谢指标检测的灵敏度较高,对结直肠癌的诊断、预防及治疗具有较高的临床应用价值。
Objective To investigate the clinical significance of combined detection of serum lipid metabolism indicator with serum CEA, CA72-4 and CA19-9 in the diagnosis of colorectal cancer. Methods Selected 100 patients with colorectal cancer (tumor group), 100 patients with colorectal polyps (polyps group) and 100 healthy subjects (control group) as the study cases, then detected the level of all subjects' serum total cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL-C), low-density lipoprotein (LDL-C) and other lipid metabolism indicators, as well as tested the level of serum carcinoembry-onic antigen (CEA), cancer antigen 72-4 (CA72-4) and carbohydrate antigen 19-9 (CA19-9) and other tumor markers with the biochemical analysis methods. Results The serum level of TC, TG, HDL-C and LDL-C in the tumor group were signifi-cantly lower than the polyps group and control group, and the difference had a statistical significance (P<0.05). The level of serum CEA, CA72-4 and CA19-9 in the tumor group was significantly higher than the polyps group and control group, and the difference had a statistical significance (P<0.05). The serum level of TC and LDL-C in the tumor group had correlation with the level of serum CEA (P<0.05), CA72-4 and CA19-9. However, the level of TG and HDL-C had no significant correlation with CEA, CA72-4 and CA19-9. Conclusion The combined detection of serum lipid metabolism indicator with serum CEA, CA72-4 and CA19-9 has a high sensitivity and the clinical value in the diagnosis , prevention and treatment of colorectal can-cer.
