摘要
目的:本文总结临床药师在降低药占比中采取的措施,为控制药占比、促进合理用药提供参考。方法:收集内蒙古自治区人民医院2016-2017年各临床科室总收入和、药品收入以及药占比扣分,进行对比分析。汇总该院临床药师2016-2017年临床不合理用药点评扣分数据,分析药品不合理应用扣分对该院降低药占比的影响。结果:该院根据各临床科室用药特点设定药占比目标值以及扣罚细则、加强合理用药培训、建立处方(医嘱)点评制度以及对部分重点监控药品实行"上锁"管理,使该院2017年药占比降到28. 89%。结论:临床药师通过发挥专业特长在控制药占比中具有明显的优势;医院奖罚管理机制是临床药师发挥作用的基础保障。通过设定目标值,每月将各临床科室药占比纳入医院质量考核当中,设定考核细则将药占比与科室绩效挂钩,提高了科室对药占比的重视程度。临床药师参与绩效管理方案的制定,然后通过医学查房、处方(医嘱)不合理用药点评以及对重点监控药品进行"上锁"管理等方式,在技术及管理层面上协助临床科室降低药占比,促进合理用药。
OBJECTIVE To summarize measures adopted to lower drug proportion by clinical pharmacists,provide a refer-ence for controlling drug proportion and promoting rational medication.METHODS The total incomes of various clinical depart-ments,drug income and score deduction of drug proportion were collected and compared between 2016 and 2017 in Inner Mongo-lia Autonomous Region People’s Hospital.Furthermore,score deduction data of drug unreasonable use were pooled to analyze theinfluence of score deduction of drug unreasonable use on reducing drug proportion.RESULTS The drug proportion of our hospi-tal in 2017 was reduced to 28.89%after setting the target value of drug proportion and detailing the regulations of penalty.CON-CLUSION The clinical pharmacists play the most important role in reducing drug proportion,such as strengthening the rationaldrug use training and establishing prescription review system.In order to strengthen key monitored drug management,the clinicalpharmacists adopt'locked'mode to restrict drug prescription.However,penalty is only a measure to reduce drug proportion,andit is the ultimate goal to promote rational drug use and reduce the medical burden of the people.The clinical pharmacists partici-pate in the development of performance management plan,then develop medical rounds and unreasonable drug evaluation to re-duce drug proportion and promote rational drug use at the technical level.
