摘要
目的:研究分析实施医保总额预付制后对某三甲医院的影响。方法:利用对比分析方法,以某医院2014年运营数据为基期,与2015年的医保总额预付对比情况数据进行分析。结果:某医院在医疗总费用、医保总量控制费用、药费、药占比、人次药费及次均费用等方面都有合理改善。结论:医院干预措施有成效,总额预付制实施效果良好,对于医院降低运营成本、规范医疗行为以及合理控制医疗费用过快增长取得了显著效果。同时,在总额预付制下医院各个科室之间的次均费用增降不一,科室之间发展不平衡。
Objectives: To analyze the impact of global budget payment in a tertiary hospital. Methods: Taking financial data in 2014 in the hospital as a baseline, data before and after implementing global budget payment were compared. Results: Total medical cost, total medical insurance payment cost, drug cost, ratio of drug to total medical cost, drug cost per capita and average cost per capita were rational improved after implementing global budget. Conclusions: Global budget payment can effectively reduce cost, standardize medical behavior and rational reduce medical expense. It also shows that there are gaps between departments on reduce rate of average medical cost per capita.
