布拉氏酵母菌预防住院新生儿继发腹泻的临床研究 被引量:13

Clinical research of using Saccharomyces boulardii to prevent secondary diarrhea in hospitalized neonates

摘要 目的了解布拉氏酵母菌对住院新生儿继发腹泻的预防效果。方法以2012年8月1日至2012年9月30日收住我院普通新生儿病房的非腹泻的患儿为研究对象,入院时随机分为观察组(常规治疗的同时加用布拉氏酵母菌)202例,日龄(8.06±6.90)d(1~28 d),男138例,女64例,感染性疾病例75例,非感性疾病127例。对照组(仅给予常规治疗)190例,日龄(7.36±6.46)d(1~27 d),男115例,女75例,感染性疾病67例,非感性疾病123例。住院期间所有患儿于入院后查大便常规及隐血、轮状病毒抗原以及其他常规检查。住院期间观察患儿大便情况,发生腹泻时立即查大便常规及隐血、轮状病毒抗原,必要时查大便培养。结果观察组和对照组患儿继发轮状病毒肠炎发生率分别为7.43%(15/202)、14.21%(27/190),两组比较差异有统计学意义(χ2=4.71,P=0.03)。腹泻持续时间分别为(3.63±1.15)d(2~7 d)、(4.48±1.16)d(2~7 d),两组比较差异有统计学意义(F=5.545,P=0.023)。所有患儿均未发生哭吵不安、皮疹、继发感染等不良反应。结论布拉氏酵母菌作为一种微生态制剂,在预防住院新生儿继发轮状病毒肠炎方面有一定效果。有临床应用价值。 Objective To explore the effect of Saccharomyces boulardii in preventing secondary diarrhea in hospitalized neonates. Methods Hospitalized infants with no diarrhea admitted during August 1,2012 to September 30, 2012 in Hunan Children's Hospital were enrolled and randomly divided into observation group and control group. Observation group( Routine treatment plus Saccharomyces boulardii) : 202 cases with average age of 8. 06 ± 6. 90 days( 1-28 days),Male: female = 138: 64,including 75 cases of infectious disease and 127 cases of non infectious disease. Control group( Routine treatment only) : 190 cases with average age of 7. 36 ± 6. 46 days( 1-27days),Male∶female = 115∶75,including 67 cases of infectious disease and 123 cases of non infectious disease. All patients received routine stool,occult blood,rotavirus antigen and other examinations at the time of admission,as well as when diarrhea occurred. Stool culture was examined when necessary. Results The rate of secondary rotavirus enteritis in observation group and control group was 7. 43%( 15 /202) and 14. 21%( 27 /190) respectively; the difference between the two groups was statistically significant( χ2= 4. 71,P = 0. 03). The duration of diarrhea in two groups was 3. 63 ± 1. 15 days( 2-7 days) and 4. 48 ± 1. 16 days( 2-7 days) respectively, with statistically significant difference( F = 5. 545,P = 0. 023). No adverse reactions such as crying spells,rash, or secondary infection were observed. Conclusion Saccharomyces boulardii,as a probiotics,has certain effect for the prevention of secondary rotavirus gastroenteritis in hospitalized neonates,and therefore has clinical value.
