摘要
目的 观察急性白血病 (AL)骨髓病变MRI表现及分布特点 ,探讨MRI诊断及对疗效的评价作用。方法 初发AL30例 ,对照组 30例 ,缓解组 12例。行腰椎、骨盆、股骨MRI检查。结果 初发AL :①腰骶椎、髂骨均匀 (2 9/30 )或结节状弥漫性长T1、长T2信号 ;股骨不均匀长T1、长T2信号 ;②ALL浸润范围于股骨头骨骺、大转子受累机会较AML大 (P <0 .0 5 )。缓解组 :病变区T1WI信号升高、T2WI信号降低。结论 AL骨髓浸润MRI表现为长T1、长T2信号 ,ALL、AML病变分布不同 ,MRI能清楚显示其特点 ,缓解期骨髓信号趋向正常。
Objective To discuss the value of MRI in diagnosing and evaluating therapeutic effect in acute leukemia (AL) by observing the MRI findings of bone marrow. Methods MRI scanning of lumbar column, pelvis and femur bone were performed in study group (30 initial AL patients manifested by marrow puncturing, including ALL 12 cases and AML 18 cases), control group (30 cases) and remission group (12 cases). Results Initial AL: ① The signal intensity on T1WI or fat suppression T2WI appeared to be homogeneous or nodulous diffuse hypointense or hyperintense at lumbosacral vertebrae and iliac bone and inhomogeneous hypointense at femur; ②The infiltrating areas of ALL was larger than AML at femur, infiltrating areas were different between ALL and AML (P<0.05). Remission group: The signal intensity on T1WI rised and on fat suppression T2WI declined. Conclusion MRI appearance of bone marrow of AL is long T1 long T2 signal intensity. ALL,AML has different distribution,MRI can clearly display these characteristics, bone marrow signal intensity of remission leukemia turn to normal.
