水蛭素对抗脑出血后脑水肿作用机制的研究 被引量:28

摘要 目的 探讨凝血酶在脑出血后脑水肿形成中的作用和水蛭素抗水肿的作用。方法 健康wistar大鼠 80只 ,随机分为 4组 ,每组 2 0只。A组 :生理盐水 +生理盐水 ;B组 :生理盐水 +水蛭素 ;C组 :血 +生理盐水 ;D组 :血 +水蛭素。采用立体定向注射方法 ,用微量注射器在右侧尾状核部位注入自体未凝血 (C、D组 )或等量生理盐水 (A、B组 ) 5 0 μl ,留置 2 0min后同部位 2次注射 1 0 μl生理盐水 (A、C组 )或等量水蛭素溶液 (B、D组 ,浓度 2u/μl)。术后第 3d断头取脑 ,测定脑水含量。 结果 A、B组比较无明显差异外 ,其余各组间均有统计学意义 (P <0 0 5 )。结论 凝血酶可以引起脑出血后脑组织水肿 ,水蛭素可以抑制凝血酶所引起的脑水肿 ,且水蛭素本身不对脑组织产生任何负面影响。 Objective To investigate the effect of hirudin on brain edema inducing by thrombin and anti-edema.Methods 80 healthy Wistar rats were distributed to 4 groups randomly(20 rats each group).Group A:infused saline plus saline;Group B:saline plus hirudin;Group C:self-body-blood plus saline;Group D:self-body-blood plus hirudin.Needles were inserted into the right caudate nuclears using stereotactic guidance.Fifty microliters of self-body-nonheparinized-fresh blood(group C and D)or saline(group A and B)were infused in 10~15 minutes and detained for 20 minutes until needle was removed.At the second infusion,10μl saline was infused into group A and C using the same coordinate and equivalent hirudin solution(concentration 2u/μl)was infused into group C and D.The rats which were measured the content of brain water were sacrificed by decapitation at the third day after the operation.Results The difference between every two groups was significant(P<0.05)except A and B.Conclusion Thrombin lead to brain edema after cerebral hemorrhage that could be restrained by hirudin,and hirudin didn't produce any negative effect on brain tissue.
