摘要
目的:评价肩袖钙化性肌腱炎应用关节镜治疗的效果及术后肩关节功能的改善情况。方法:将2016年9月-2018年5月收治的23例肩袖钙化性肌腱炎患者纳入研究,均接受关节镜治疗。记录手术情况,展开6个月随访,对比手术前后患者疼痛、肩关节功能情况。结果:平均手术时间为(142.53±20.25)min,平均术中出血量为(198.75±32.42)ml,平均住院时间为(6.52±1.24)d。术后X线检查显示,钙化点残留2例(8.70%)。术后关节活动度明显大于术前,差异有统计学意义(P<0.05)。术前VAS评分为(7.28±1.14)分,术后6个月VAS评分为(2.07±0.55)分,术后VAS评分显著低于术前,差异有统计学意义(t=19.740,P<0.05)。术后疼痛情况优于术前,未见中重度疼痛,差异有统计学意义(P<0.05)。术前Constant评分为(40.86±9.72)分,术后6个月Constant评分为(89.75±5.45)分,肌力、疼痛、日常生活活动、主活动范围评分均显著高于术前,差异均有统计学意义(P<0.05)。术后6个月,治疗优良率为95.65%(22/23)。结论:肩袖钙化性肌腱炎患者使用关节镜治疗可有效清除钙化,肩关节功能恢复良好。
Objective:To evaluate the effect of arthroscopic on rotator cuff calcific tendinitis and the improvement of shoulder joint function after operation.Method:A total of 23 patients with rotator cuff calcific tendonitis admitted from September 2016 to May 2018 were enrolled in the study,and all received arthroscopic treatment.The operation was recorded and followed up for 6 months to compare the pain and shoulder joint function of the patients before and after the operation.Result:The mean operative time was(142.53±20.25)min,the mean intraoperative blood loss was(198.75±32.42)ml,and the average hospital stay was(6.52±1.24)days.Postoperative X-ray examination showed that 2 cases(8.70%) had calcified spots.The range of motion of joints after operation was significantly higher than that before operation,and the difference was statistically significant(P<0.05).The preoperative VAS score was(7.28±1.14)points,and VAS score was(2.07±0.55)points at 6 months after operation.VAS score after operation was significantly lower than that before operation,and the difference was statistically significant(t=19.740,P<0.05).Postoperative pain was better than preoperative pain,no moderate or severe pain was observed,and the difference was statistically significant(P<0.05).The Constant score was(40.86±9.72)points before operation,and at 6 months after operation was(89.75±5.45)points.The scores of muscle strength,pain,daily activities and the main range of activity were significantly higher than those before operation,and the differences were statistically significant(P<0.05).6 months after operation,the excellent and good rate of treatment was 95.65%(22/23).Conclusion:Arthroscopic treatment of patients with rotator cuff calcific tendinitis can effectively remove calcification and shoulder joint function recovered well.
