摘要
目的评价老年大面积急性心肌梗死(AMI)患者行冠状动脉介入治疗(PCI)血运重建中早期辅以主动脉球囊反搏(IABP)治疗的安全性及有效性。方法回顾性分析65例大面积急性心肌梗死(AMI)患者行冠状动脉介入治疗(PCI)的老年患者,其中30例早期接受IABP治疗设为实验组,35例未接受IABP治疗患者为对照组。比较实验组患者使用IABP前后血流动力学各项指标及心功能的变化;比较两组患者治疗效果及术后主要心血管不良事件发生率。结果实验组在使用IABP后血流动力学各项指标及CI/EF较使用前有显著改善(P<0.05或P<0.01);两组患者治疗效果比较,差异有统计学意义(P<0.05或P<0.01);两组患者术后主要心血管不良事件发生率的比较,差异有统计学意义(P<0.05)。结论老年大面积AMI患者行PCI早期给予IABP的辅助使用,能降低围手术期死亡率,减少术后主要心血管不良事件的发生。IABP安全性及有效性高,值得临床推广。
Objective To evaluate the safety,effectiveness and prognosis of Intra-aortic balloon pump(IABP)during the procedure of percutaneous coronary intervention(PCI)for old patients with large area of acute myocardial infarction(AMI).Methods We analyzed a consecutive series of 65 patients with large area of AMI before elective PCI retrospectively.These patients were classified into two groups according to whether they received IABP treatment during perioperative period:30patients in IABP group and 35 in control group.The effectiveness and occurence of major adverse cardiovascular events(MACE)were compared between the two groups.Statistical analysis was performed using analysis of variance,with P <0.05 indicating significance.Results In IABP group,hemodynamic indicators,CI/EF,the effectiveness and occurence of MACE were statistically significant in two groups(P <0.05).Conclusion Early application of IABP for old patients with large area of AMI before PCI is safe and effective,which can reduce perioperative mortality and incidence of MACE,and is valuable for general application.
