摘要
目的:探讨下腔静脉变异及布-加综合征对肾癌患者手术治疗的影响及手术要点。方法:报告1例右侧肾癌伴下腔静脉变异及布-加综合征患者的临床资料,结合相关文献复习并讨论。结果:患者,男,60岁。因"阴囊水肿"于外院行影像学检查发现右肾区占位入院,入院查体发现有明显的布-加综合征合并下腔静脉畸形。患者顺利接受后腹腔镜下右肾根治性切除术,术后病理诊断为肾脏透明细胞癌。结论:下腔静脉是右侧腹膜后病变腹腔镜手术的重要解剖标志,术前详细的体格检查及影像学检查对手术的顺利实施具有重要的临床意义。下腔静脉造影对下腔静脉变异的诊断及手术策略选择具有重要的参考价值。
Objective:To investigate the impact of inferior vena cava variation and Budd-Chiari syndrome on the surgical treatment of renal cancer.Methods:The clinical data of one patient with right kidney cancer with inferior vena cava variation and Budd-Chiari syndrome were reported,and related literatures were reviewed.Results:The patient who complained 'scrotal edema' and showed imaging findings of the right renal mass,was revealed as BuddChiari syndrome with inferior vena cava malformation.The patient underwent retroperitoneal laparoscopic radical nephrectomy.Postoperatively,the definite diagnosis of renal clear cell carcinoma was made pathologically.Conclusions:The inferior vena cava is an important anatomical sign of laparoscopic surgery for right retroperitoneal lesions.Detailed preoperative physical and imaging examinations are of great clinical significance for the operation.Inferior vena cava angiography plays an important role in the diagnosis and surgical strategy of inferior vena cava variation.
