摘要
目的进一步探讨专家对抑郁症肝郁脾虚、心脾两虚证证候标准的认识。方法根据文献资料及第一轮专家问卷分析结果 ,编制抑郁症常见中医证候标准第二轮专家问卷 ,向全国范围内 1 0 2名专家发送问卷调查 ,采用德尔菲评价法进行问卷分析。结果抑郁症肝郁脾虚证中情绪抑郁、多愁善虑等 6项 ,心脾两虚证中情绪低落、面色萎黄等 7项各指标的等级和大于 1 5 0 ,均数大于 1 .5 ,满分比大于 0 .5 ,变异系数小于0 .4。结论情绪抑郁、多愁善虑、倦怠乏力、舌质淡红或淡白、纳差、脉细或弦细等证候指标可作为抑郁症肝郁脾虚证的主症 ,情绪低落、面色萎黄、舌质淡胖或有齿痕、脉沉细或细弱、心悸、疲怠乏力、纳差等证候指标可作为抑郁症心脾两虚证的主症。
Objective To gain more knowledge on the pattern standards of Liver depression-Spleen deficiency and dual deficiency of the Heart and Spleen from the Specialists all over the country.Methods On the basis of published literature studying and the outcome of analysis made on the first turn of questionnaires,the second turn of questionnaires was prepared to investigate the pattern standards of depression further.They were distributed to 102 experts in this field all over the country.Then, the questionnaires were analysed with the method of Delphi Measurement. Results The pattern of Liver depression and Spleen deficiency for this lesion was mainly manifested as 6 kinds of symptom like mental depression,sentiment and so on,and the one of dual deficrency of the Heart and Spleen was mainly manifested as 7 kinds of symptom like mental depression,withered-yellow facial complexion and so on.No matter which case it was,it should be seen with the sum of rank larger than 150,the mean number more than 1.5,the ratio of full scores higher thom 0.5 and the coefficient of variation less than 0.4.Conclusions The main symptoms for the pattern of Liver depression and Spleen deficioncy in this lesion are mental depression,sentiment,fatigue,reddish or pale tongue, reduced food intake,fine or fine stringlike pulse,while those for the pattern of dual deficiency of the Heart and Spleen may be mental depression,withered-yellow facial complexion,pale-enlarged tongue with tooth marks, deep fine or fine weak pulse,palpitation,fatigue and reduced food intake.
