摘要
目的探讨耳软骨构建三角支架在鼻尖成形中的应用。方法切取一侧耳甲腔软骨和耳甲艇软骨,将耳甲腔软骨切成3条,第1条宽4~8 mm,第2条宽2~4 mm,第3条宽2~4 mm,第1条软骨和第2条软骨相对缝合构成中柱移植物,第3条用做支撑柱移植物;耳甲艇软骨用于'帽子'移植物和'盾牌'移植物。将中柱移植物缝合固定于鼻中隔尾端的前端,支撑柱移植物缝合固定于中柱移植物的上端及鼻中隔尾端的上部。为进一步调节鼻尖高度和形态,在移植中柱的上方做1~3层厚的由耳甲艇软骨构成的'帽子'移植物及前上部由耳甲艇软骨条形成的'盾牌'移植物。结果本组68例患者获随访3~36个月,鼻部切口均愈合良好,无血肿、感染发生。鼻尖形态均获明显改善。2例耳甲腔处小血肿,机化后逐渐吸收,未影响耳郭轮廓。结论采用耳软骨构建三角支架成形鼻尖是一种安全可行的良好的鼻尖成形方法。
Objective To explore the application of a triangular bracket composed of auricular cartilage in nasal tip rhinoplasty.Methods The cavum conchae and cymba conchae cartilage of one ear was cut.The cavum conchae cartilage was sliced into 3 strips of different widths.The first was from 4 to 8 mm,the second was from 2 to 4 mm,and the third was from 2 to 4 mm.The first and second strip of cartilage was sutured as a middle column graft,and the third was used as support a column graft.The cymba conchae cartilage was applied as the onlay tip graft and shield graft.The middle column graft was sutured at the front of the end of the nasal septal cartilage,and the support column graft was sutured and fixed to the upper of the middle column graft and the end of the nasal septal cartilage.To further adjust the height and shape of the nasal tip,the onlay tip graft was composed of 1 to 3 layers of cymba conchae cartilage.It was made above the middle column graft,and a shield graft formed by a strip of the cymba conchae cartilage was made in the anterior superior part of the middle column graft.Results A total of 68 patients were followed up from 3 to 36 months.All nasal incisions healed well,and no hematoma or infection occurred.The morphology of the nasal tips was obviously improved.No patients were unsatisfied or required reoperation.Small hematomas in the cavum conchae occurred in 2 patients but gradually absorbed after organization.The contour of the auricle was unaffected.Conclusion Applyinga triangular bracket composed ofauricular cartilage toformthe nasal tip was a safe and feasible method.
