肺泡灌洗对肺炎支原体重症肺炎儿童通气指标的影响 被引量:10

Effect of alveolar lavage on the ventilation index of the children with severe mycoplasma pneumoniae

摘要 目的探讨肺泡灌洗对肺炎支原体重症肺炎儿童通气指标的影响。方法选取2017年2月至2019年4月来杭州市儿童医院救治的肺炎支原体重症肺炎儿童180例为研究对象。其中在急性期进行支气管检查并行肺泡灌洗治疗的患者为研究组(n=80),未进行支气管镜肺泡冲洗的患者为对照组(n=100);干预1周后观察分析两组研究对象的临床治疗效果、症状、体征及通气指标。结果治疗1周后,研究组临床治疗有效率(82.50%)高于对照组(26.00%),χ~2=4.89,P=0.03<0.05;研究组患者的胸片好转率(26.25%)高于对照组(11.00%),χ~2=5.89,P=0.02;研究组患儿的平均热程和住院时间均较对照组缩短(t值分别为2.21、2.20,均P=0.03<0.05);研究组患者的通气指标最大肺活量(FVC)、一秒用力呼气容积(FEV1)、PEF明显高于对照组(t值分别为5.08、4.26,均P<0.01;PEF的t=2.52,P=0.02<0.01);研究组患者的最大呼气中期流量(FEF25%、FEF50%、FEF75%),最大呼气中期流速(FEF25%~75%)均明显高于对照组(t值分别为3.09、3.11、8.53、9.42,均P<0.01)。结论肺泡灌洗能明显改善重症肺炎支原体肺炎儿童的通气功能,其治疗效果在临床上非常显著。 Objective To investigate the effect of alveolar lavage on ventilation index in children with severe mycoplasma pneumoniae.Methods A total of 180 children with pneumonia and mycoplasma pneumoniae who were treated in Hangzhou Children’s Hospital during the period from February 2017 to April 2019 were selected as subjects.The patients who underwent bronchial examination and alveolar lavage in the acute phase were the study group(n=80),and the patients who did not undergo bronchoscopy alveolar lavage were the control group(n=100).After 1 week of intervention,the clinical treatment effects,symptoms,signs and ventilation indexes of the two groups were observed and analyzed.Results After one week of treatment,the clinical treatment efficiency of the study group(82.50%)was higher than the control group(26.00%)(χ~2=4.89,P=0.03);the chest radiograph improvement rate of the study group(26.25%)was higher than that of the control group(11.00%)(χ~2=5.89,P=0.02);the mean heat history and hospital stay of the study group were shorter than those of the control group(t=2.21,P=0.03;t=2.20,P=0.03);The ventilation indexes FVC,FEV1,and PEF of the study group were significantly higher than those of the control group(t=5.08,P<0.01;t=4.26,P<0.01;t=2.52,P=0.02);the maximum expiratory mid-flow rate(FEF25%,FEF50%,FEF75%)and the maximum expiratory mid-flow rate(FEF25-75%)were significantly higher in the study group than in the control group(t=3.09,3.11,8.53,9.42,P<0.01).Conclusions Alveolar lavage can significantly improve the mycoplasma pneumonia children’ventilation function of the children with mycoplasma pneumoniae,and the clinical treatment effect is remarkable.
