摘要
目的探索抗炎因子白细胞介素-4(IL-4)、促炎细胞因子白细胞介素-6(IL-6)以及肿瘤坏死因子(TNF-α)浓度与新生儿坏死性小肠结肠炎(NEC)临床分期的关系。方法选择2015年7月至2018年5月被浙江省慈溪市人民医院收治的NEC患儿124例为观察组(NEC组),再选择100例健康患儿为对照组,比较不同NEC分期IL-6、TNF-α以及IL-4的差异,并采用16srDNA PCR产物高通量测序技术对比NEC患儿肠道内菌群丰富度与细胞因子浓度的相关性。结果 (1)NEC组IL-6、TNF-α和IL-4浓度均显著高于对照组(t值分别为3.15、9.55、6.02,均P<0.01);(2)NEC组各期IL-6、TNF-α、IL-4浓度逐渐升高,组内比较差异有统计学意义(F值分别为5.59、10.78、8.22,均P<0.01),其中Ⅱ期高于Ⅰ期(t值分别为5.96、7.81、6.00,均P<0.05),Ⅲ期高于Ⅱ期(t值分别为6.95、5.10、5.01,均P<0.05);(3)TNF-a和IL-6浓度与Shannon指数呈负相关性(r值分别为-0.56、-0.35,均P<0.05),与Simpson指数呈正相关(r值分别为0.47、0.81,均P<0.05)。结论抗炎或促炎细胞因子浓度与NEC的临床分期及肠道菌群丰富度密切相关,可作为NEC病情诊断的辅助指标。
Objective To explore the relationship between anti-inflammatory cytokine interleukin 4(IL-4),pro-inflammatory cytokine interleukin 6(IL-6),tumor necrosis factor-α(TNF-α)and clinical staging of neonatal necrotizing enterocolitis(NEC).Methods From July 2015 to May 2018,124 cases of NEC children admitted to People’s Hospital of Cixi City were selected as observation group(NEC group).Another 100 healthy children were selected as control group.The differences in IL-6,TNF-αand IL-4 in different NEC stages were analyzed.The correlation between intestinal flora richness and cytokine concentration was analyzed by high throughput sequencing of 16 srDNA PCR products.Results The concentrations of IL-6,TNF-αand IL-4 in NEC group were higher than in the control group,and the differences were statistically significant(t value was 3.15,9.55 and 6.02,respectively,all P<0.05).The concentrations of IL-6,TNF-αand IL-4 in different stages in NEC group increased gradually,and there was significant difference within group(Fvalue was 5.59,10.78 and 8.22,respectively,all P<0.01).The levels of IL-6,TNF-αand IL-4 in stage II were higher than those in stage I(t value was 5.96,7.81 and 6.00,respectively,all P<0.05),and in stage III were higher than in stage II(t value was 6.95,5.10 and 5.01,respectively,all P<0.05).TNF-αand IL-6 concentrations were negatively correlated with Shannon index(r value was-0.56 and-0.35,respectively,both P<0.05)but positively correlated with Simpson index(r value was 0.47 and 0.81,respectively,both P<0.05).Conclusion The concentration of anti-inflammatory or pro-inflammatory cytokines is closely related to clinical stage and intestinal flora richness of NEC,and can be used as an auxiliary index for the diagnosis of NEC.
