新生儿海绵窦外侧壁的显微外科解剖 被引量:2

摘要 目的 了解新生儿海绵窦外侧壁的解剖学结构,为海绵窦外侧壁手术入路提供基础资料。方法 在显微镜下对20例(40侧)新生儿海绵窦外侧壁结构进行解剖和观察。结果 海绵窦外侧壁深层不完整者占62.5%,均在Mullan三角前部或Parkinson三角后部;Mullan三角的内侧边长5.5±1.02mm,外侧边长7.42±2.07mm,底边长4.43±1.21mm;Parkinson三角的内侧边6.45±1.82mm,外侧边长三角外侧边7.60±1.92mm,底边长2.72±0.53mm;滑车神经在海绵窦段分三型,即交叉型,凸型和直型。结论 滑车神经在海绵窦段行程类型不同可影响Parkinson三角的显露。海绵窦外侧壁深层不完整除与浅表小隙形成有关外,与穿行神经间距有关,与胚胎发育及遗传因素有关;选择海绵窦外侧壁入路时Parkinson三角最常用,在海绵窦前半部可联合应用Mullan三角。 Objective To understand the anatomic structure of the paries lateralis of cavernous sinus in newborns.to provide the basic materials for the operation. Methods The paries lateralis structures of cavernous sinus in 20 newborns were dissected and analysed through the microscope. Results The stratum profundum of paries lateralis in cavernous sinus was incomplete (62. 5% ), they were in the front of the Mullan triangle or in the back of Parkinson triangle. The medial Mullan triangle was 5.51 ±. 20mm long, the lateral 7. 42±2.07mm, the basialis 4.43±1.21mm. The Parkinson triangle was 6. 45 ?1.82mm long, the lateral 7.60±1.92mm, the basialis2. 72± 0.53mm. The pathetic nerve could be divided into three types in the section of cavernous sinus:cross - over,convex and straight. Conclusion The different types of the pathetic nerve in the section of cavernous sinus can effect the appearance of the Parkinson triangle. The incomplete stratum profundum of paries lateralis in cavernous sinus is not only related to the formation of superficial compartment , but also the internal of perforating nerve, embryonic development and hereditary factor. The Parkinson triangle is ofen used in the operation,and it can combine with Mullan triangle when the operation is in the half front of the cavernous sinus.
