摘要
目的:探讨集对分析在银屑病代表方剂用药规律研究中的应用,以求进一步提高疗效。方法:先对文献中报导的治疗银屑病的9个代表方剂所获得的疗效用4元联系数表示;再对疗效4元联系数中的'治愈''显效''好转''无效'分别取值为1,0.666(-0.333),0.333(-0.666),0(-1),据此计算9个代表方剂的总有效率以及与理想完美疗效的距离,综合有效率大的优于综合有效率小的,与完美疗效距离短的优于距离大的,从而确定9个代表方剂的综合优劣排序;讨论排在前3位的代表方剂之药理作用,找出治疗银屑病的中药应用规律。结果:综合疗效排在第1、2、3位的依次是土槐菝葜汤,清凉解毒汤,凉血活血汤,3个汤剂中无共用药,任意两个方剂的共用药有生地、土茯苓、白鲜皮、赤芍、板蓝根、当归、紫草、茜草、槐花、丹皮,最优方剂的综合疗效与理想的完美疗效之间仍有一半距离,建议在综合疗效排序第一的方剂土槐菝葜汤的基础上增加鸡血藤、雷公藤、蜈蚣、羚羊角、大黄,以获得更高的疗效。结论:可以在中医治疗银屑病的多个方案疗效综合评价中应用基于集对分析的加权疗效曲线和总有效率计算,提高银屑病中医中药治疗效果的任务仍很艰巨。
Objective:To explore the application of set pair analysis in the study of the medication law of representative agents in psoriasis so as to further improve the curative effect.Methods:First,the curative effect of 9 representative formulae in the treatment of psoriasis suggested in the literature was expressed by four-element connection number.Then,the values of'cure','markedly effective','improvement'and'invalid'in the four-element connection number were 1,0.666(-0.333),0.333(-0.666)and 0(-1).On the basis,we calculated the total effective rate of nine representative prescriptions and the distance away from the perfect curative effect.The prescriptions with larger comprehensive efficient was better than that with small comprehensive efficient and those with short distance away from the perfect effect was better than those with long distance and then the order of the nine representative prescriptions was determined.We discussed the top three representative prescriptions pharmacological action to find out the application law of Chinese medicine treating psoriasis.Results:The top three prescriptions according to the comprehensive efficacy were Tuhuaihua Decoction,Qingliang Jiedu Decoction and Liangxue Huoxue Decoction,respectively.They have no common drugs.In any two,the common drugs included Shengdihuang(radix rehmanniae recens),Tufuling(rhizome smilacis),Baixianpi(cortex dictamni),Chishao(radix paeoniae),Banlangen(radix isatidis),Danggui(angelica),Zicao(radix lithospermum),Qiancao(radix rubiae).The comprehensive effect of the optimal formula still had half the distance away from the ideal perfect curative effect.Conclusion:The comprehensive evaluation based on set pair analysis of the weighted efficacy curve and the calculation of total effective rate can be used in traditional Chinese medicine treatment for psoriasis and to improve the effect of Chinese medicine treatment of psoriasis is still arduous.
