血清白介素-2受体水平在慢性肾脏病患者中的变化及影响因素

摘要 目的观察慢性肾脏病(CKD)1-5期非透析患者血清白介素-2受体(sIL-2R)的水平及其影响因素。方法选取CKD患者74例,分为CKD早期组[eGFR≥60ml·min^(-1)·(1.73m^2)^(-1)]、CKD中晚期组[eGFR<60ml·min^(-1)·(1.73m^2)^(-1)],测定各组sIL-2R水平,分析其与一般生化指标间的关联。进行CKD合并糖尿病者的亚组分型,分析其与sIL-2R水平间的关联。结果 CKD早期组年龄、sIL-2R、尿酸、尿素氮、中性粒细胞计数、BNP、肌酐、PTH水平显著低于CKD中晚期组,而淋巴细胞计数、血色素、血小板、空腹血糖、白蛋白、eGFR水平则显著高于CKD中晚期组(P<0.05)。CKD患者的sIL-2R水平同CKD患者年龄、PTH、血磷、BNP水平呈正相关(r分别为0.235、0.360、0,291、0.428,P<0.05),同淋巴细胞计数、血红蛋白、血钙、eGFR呈负相关(r分别为-0.39、-0.624、-0.273、-0.561,P<0.05)。多元线性相关分析提示,淋巴细胞计数、血红蛋白、PTH水平是血清IL-2R的独立影响因素。sIL-2R在CKD合并糖尿病与未合并糖尿病者间未见明显差异(P>0.05)。结论不同分期的CKD患者血清白介素-2受体水平存在显著差异。CKD患者的sIL-2R水平随CKD患者肾功能的减退而逐步升高。CKD患者的sIL-2R水平同CKD患者是否罹患糖尿病间无明显相关性。定期检测CKD患者的sIL-2R水平有助于判断(KD患者的微炎症状态。 Objective To investigate the expression of serum Interleukin-2 receptor(IL-2R) in chronic kidney disease(CKD) people and its factors.Methods A total of 74 patients with CKD were involved in this study.The levels of serum IL-2Rwere measured by ECL assay.Make clear the relationship with routine biochemical tests and the difference between CKD people with diabetes and without diabetes.Results According to the progression of glomerular filtration rate(GFR).the patients with CKD were divided into two groups;group CKD1-2[eGFR≥60ml · min^(-1) ·(1.73m^2)^(-1)],group CKD3-5N[eGFR<60ml · min^(-1)·(1.73m^2)^(-1)].The levels of serum IL-2R were relatively higher in group CKD3-5a(P<0.05).There wereno difference between CKD people with diabetes and without diabetes(P>0.05).The level of serum IL-2Rwas positively correlated with age.PTH,P,BNP levels in CKD and negatively with lymphocyte count,hemoglobin.Ca.eGFR levels by Pearson correlation analysis.Conclusion The level of serum IL-2R was negatively correlated with the function of kidney.There was significantly difference between the early and late stage of CKD people.There wereno difference between CKD people with diabetes and without diabetes.Serum IL-2R maybe play an important role in the progression of renal function defect of CKD with diabetes.It is advised to detect the serum level of IL-2R regularly to help make sure the micro-inflammation status of CKD people.
