不同术式治疗脊柱胸腰段压缩性骨折的近期及长期疗效对比 被引量:15

Recent and long-term efficacy of different surgical in the treatment of thoracolumbar vertebral fracture

摘要 目的 :探讨椎体后凸成形术(percutaneous kyphoplasty,PKP)、微创椎弓根钉内固定技术治疗脊柱胸腰段压缩性骨折的近期、长期疗效。方法 :根据随机对照分组原则,80例患者分为A组(n=40)和B组(n=40),A组患者接受椎体后凸成形术治疗,B组采用微创椎弓根钉内固定术治疗,所有患者获3年随访,将两组患者的近期、长期疗效进行对比分析。结果 :两组的手术时间、术中出血量均无统计学差异,B组住院时间短于A组;VAS评分的组间对比无显著性差异;术后长期随访结果显示,A组伤椎高度丢失量明显大于B组;组间并发症发生率无显著差异。结论 :对于脊柱胸腰段压缩性骨折的术式选择应综合考虑各方面情况,微创椎弓根钉内固定技术的长期疗效更为显著,值得临床进一步推广应用。 Objective To investigate the recent and long-term efficacy of PKP and vertebral screw internal fixation in the treatment of thoracolumbar vertebral fracture.Methods 80 patients were divided into A group(n=40) and B group(n=40).Patients of the A group took PKP while B group took internal fixation.All patients were followed up for an average of 3 years.Results There has no significant different in operative time,blood loss,VAS scores between the two groups.However,hospital stay time of B group was significant shorter than that of A group.The vertebrae height loss of A group was much higher than B group.Complication rate showed no significant difference between groups.Conclusion Each aspect should be considered in treating thoracolumbar vertebral fracture.Vertebral screw internal fixation have show obvious advantage in long-term efficacy and is worthy of application.
