摘要
目的:分析直径≤3cm周围型小细胞肺癌(SCLC)的CT影像学表现,提高对SCLC的诊断水平。方法:回顾性分析2010年5月至2017年8月间经病理证实直径≤3cm周围型SCLC影像及临床资料,以周围型肺腺癌(ADC)作为对照组,比较分析两组病例一般资料(年龄、性别及吸烟史)、CT征象[病灶部位、大小、密度、形态、分叶征、周围磨玻璃影、毛刺征、钙化、空泡/空洞征、支气管空气征、血管集束征、胸膜凹陷征、胸膜侵犯、胸腔及心包积液、肺内播散(同肺叶、不同肺叶)、淋巴结肿大(短轴>1.0cm)、分期]之间的差异。用多因素Logistic回归分析SCLC的影响因素。结果:符合条件SCLC 35例,ADC 244例。Logistics回归分析显示:性别(OR=4.852)、吸烟(OR=3.109)、形态(OR=10.717)、分叶征(OR=6.165)、周围磨玻璃影(OR=0.016)、毛刺征(OR=0.048)、支气管空气征(OR=0.051)及胸膜凹陷征(OR=0.128)与SCLC相关(P<0.05)。结论:与ADC比较,SCLC以男性、吸烟、圆形/类圆形及分叶征多见,周围磨玻璃影、毛刺征、空气支气管征及胸膜凹陷征少见。
Objective:To analyze CT imaging features of small cell lung cancer(SCLC)with the diameter less than 3 cm,so as to improve the diagnostic level of SCLC.Methods:A retrospective analysis was conducted by analyzing imaging and clinical data of the peripheral SCLC confirmed by pathological method(experimental group)and lung adenocarcinoma(ADC)(control group)with the diameter less than 3 cm between May 2010 and August 2017.The comparative analysis between the two groups of patients with general information(age,gender,and smoking history),CT signs differences including lesion location,size,density,shape,lobulation,outside of consolidation ground glass shadow,spiculation,calcification,cavitation/bubble sign,air bronchogram,pleural indentation,vessel convergence,pleural indentation,pleural invasion,pleural and pericardial effusion,associated findings(primary tumor lobe or nontumor lobes),enlarged lymph node,staging,etc.The correlation between imaging findings and peripheral SCLC was analyzed by multifactor Logistic regression method.Results:There were 35 cases of SCLC and 244 cases of ADC.Logistics regression analysis showed that SCLC was significantly associated with the gender(OR=4.852),smoking(OR=3.109),shape(OR=10.717),lobulation(OR=6.165),ground glass shadow(OR=0.016),spiculation(OR=0.048),air bronchogram(OR=0.051),and pleural indentation sign(OR=0.128)(P<0.05).Conclusion:Compared with those of adenocarcinoma patients,the clinical data and CT scans of patients with SCLC less than 3.0 cm in size showed that male,smoking,round and lobulation were more frequent,whereas surrounding ground glass,spiculation,air bronchogram and pleural indentation signs were observed less frequently.
