摘要
目的研究脐血白细胞介素1β(IL-1β)、白细胞介素6(IL-6)、白细胞介素8(IL-8)、白细胞介素10(IL-10)、肿瘤坏死因子-α(TNF-α)与母体、胎儿炎症反应及早产预后的相关性。方法选取2015年1月至2016年1月在我院妇产科就诊的早产孕妇及早产儿各93例作为研究对象。比较母体炎症反应不同时期的脐血细胞因子水平,胎儿炎症反应不同分型的脐血细胞因子水平,不同早产预后的脐血细胞因子水平,利用Spearman相关性分析法分析脐血细胞因子与母体或胎儿炎症反应及早产预后的相关性。结果母体炎症反应≥2期者的IL-1β、IL-6、IL-8及TNF-α的水平明显高于无炎症反应与1期者,IL-10水平明显低于无炎症反应与1期者,而1期的IL-1β、IL-6、IL-8及TNF-α的水平明显高于无炎症反应者,IL-10明显低于无炎症反应者,差异均有统计学意义(P<0.05);胎儿2型炎症反应的IL-1β、IL-6、IL-8及TNF-α的水平明显高于无炎症反应与1型者,IL-10水平明显低于无炎症反应与1型者,而1型的IL-1β、IL-6、IL-8及TNF-α的水平明显高于无炎症反应者,IL-10水平明显低于无炎症反应者,差异均有统计学意义(P<0.05);预后不良的早产儿脐血IL-1β、IL-6、IL-8及TNF-α水平均明显高于预后良好者,而IL-10水平明显低于预后良好者,差异均有统计学意义(P<0.05);根据Spearman法评价相关性可知,IL-1β、IL-6、IL-8及TNF-α分别与母体或胎儿炎症反应及早产预后均呈正相关(P<0.05),而IL-10与母体或胎儿炎症反应以及早产预后呈负相关(P<0.05)。结论脐血IL-1β、IL-6、IL-8及TNF-α水平分别与母体或胎儿炎症反应及早产预后呈正相关,而IL-10与母体或胎儿炎症反应及早产预后呈负相关。
Objective To study the correlation of interleukin-1β(IL-1β), interleukin-6(IL-6), interleukin-8(IL-8), interleukin-10(IL-10), TNF-α with maternal, fetal inflammatory response and prognosis of preterm birth.Methods From January 2015 to January 2016, 93 pregnant women with preterm birth and the preterm infants in Department of Obstetrics and Gynecology in our hospital were selected as the study subjects. The umbilical cord blood cytokine levels in different periods of maternal inflammatory response, umbilical cord blood cytokine levels in different types of fetal inflammatory response, and the levels in different prognosis of preterm birth were compared. Spearman method was used to analyze the correlation between umbilical cord blood cytokine and maternal or fetal inflammatory response, prognosis of premature birth. Results IL-1β, IL-6, IL-8 and TNF-α levels in the period of maternal inflammatory response ≥2 stage were significantly higher than those of non-inflammatory response and 1 stage, while IL-10 level in the period of maternal inflammatory response ≥2 stage was significantly lower. The levels of IL-1 β, IL-6, IL-8and TNF-α in the 1 stage were significantly higher than those of non-inflammatory reaction, while IL-10 was significantly lower(P<0.05). IL-1β, IL-6, IL-8 and TNF-α levels in type 2 fetal inflammatory response were significantly higher than those in non-inflammatory reaction and type 1 fetal inflammatory response, while IL-10 levels in type 2 fetal inflammatory response were significantly lower. IL-1 β, IL-6, IL-8 and TNF-α levels in type 1 fetal inflammatory response were significantly higher than those of non-inflammatory reaction, while IL-10 levels were significantly lower(both P<0.05). IL-1 β, IL-6, IL-8 and TNF-α were significantly higher in the umbilical cord blood of preterm infants with poor prognosis than those with good prognosis, and IL-10 levels were significantly lower than those with good prognosis(P<0.05). According to Spearman analysis, IL-1β, IL-6, IL-8 and TNF-α were positively correlated with maternal or fetal inflammatory reaction and prognosis of premature birth(P<0.05), and IL-10 was negatively correlated with maternal or fetal inflammatory reaction and prognosis of premature birth(P<0.05). Conclusion IL-6, IL-8, IL-1βand TNF-α levels are positively correlated with maternal or fetal inflammatory response as well as prognosis of premature birth, while IL-10 is negatively correlated with maternal or fetal inflammatory response and prognosis of premature birth.
