摘要
目的探讨产前多普勒超声诊断单脐动脉、持续性右脐静脉胎儿与畸形的关系。方法选取2011年1月至2015年5月在我院进行产前多普勒超声诊断的胎儿220例,根据超声诊断结果将其分为单脐动脉胎儿160例(A组)和持续性右脐静脉胎儿60例(B组),分析并比较两组胎儿合并畸形的情况。结果 A组胎儿中有46例合并畸形,检出率为28.8%;46例合并畸形胎儿中15例为心脏异常,11例为泌尿生殖系统异常,10例为神经系统异常,其他为骨骼及肢体异常、胸部异常及颜面部异常,分别为5例、3例与2例。B组胎儿中15例合并畸形,检出率为25.0%;15例合并畸形胎儿中4例为心脏异常,4例为骨骼及肢体异常,3例为神经系统异常,2例为消化系统异常,1例胸部异常,1例为泌尿生殖系统异常。结论患有单脐动脉、持续性右脐静脉的胎儿具有较高合并畸形的概率,临床上进行产前超声检查对预防胎儿畸形,提高新生儿出生质量具有非常重要的价值。
Objective To investigate the relationship between Doppler ultrasound prenatal diagnosis of single umbilical artery, persistent right umbilical vein and fetal malformation. Methods A total of 220 fetuses who underwent Doppler ultrasound prenatal diagnosis in our hospital from Jan. 2011 to May 2015 were selected. According to the results of ultrasonic diagnosis, the fetuses were divided into 160 cases of single umbilical artery(group A) and 60 cases of persistent right umbilical vein(group B). The fetal malformation was analyzed and compared between the two groups. Results A total of 46 cases showed fetal malformation in group A, and the detection rate was 28.8%, including cardiac abnormalities in 15 cases, urogenital system exceptions in 11 cases, nervous system abnormalities in 10 cases,bone and limb abnormalities in 5 cases, abnormal chest in 3 cases, facial abnormalities in 2 cases. Fifteen cases showed fetal malformations in group B, and detection rate was 25.0%, including cardiac abnormalities in 4 cases, bone and limb abnormalities in 4 cases, nervous system abnormalities in 3 cases, abnormal digestive system in 2 cases, chest abnormalities in 1 case, abnormal urogenital system in 1 case. Conclusion Fetuses with single umbilical artery, persistent right umbilical vein have a higher risk of fetal malformation. Prenatal ultrasonography is of great clinical value to prevent fetal malformation and improve the quality of the newborn.
