核磁共振成像技术诊断膝关节前交叉韧带断裂的临床研究 被引量:40

摘要 目的评价核磁共振成像技术诊断膝关节前交叉韧带断裂的临床准确性。方法随机抽取2013年4月至2016年1月我院关节创伤外收治的科膝关节前交叉韧带断裂患者60例,以关节镜检查结果作为标准,通过核磁共振成像技术对患者进行诊断,将诊断结果与医师单纯临床诊断结果进行对比。结果医师通过临床诊断,有48例患者膝关节前交叉韧带断裂(41例部分断裂,7例全部断裂),12例患者正常。部分和全部断裂平均准确率为79.78%,假阴性率为23.5%。核磁共振成像技术诊断患者,有53例患者膝关节前交叉韧带断裂(45例部分断裂,8例全部断裂),7例患者正常。部分和全部断裂平均准确率为88.56%,假阴性率为13.7%。核磁共振成像技术诊断准确率高于医师单纯临床诊断,差异具有统计学意义(P<0.05)。结论核磁共振成像技术诊断前交叉断裂较单纯临床诊断具有更高的准确性,但仍存在漏诊的可能,需结合医师临床综合分析,从而提高诊断的准确率。 Objective To evaluate the clinical accuracy of magnetic resonance imaging(MRI) in the diagnosis of the knee anterior cruciate ligament(ACL) rupture. Methods Sixty patients with knee ACL rupture in Department of Joint Trauma Surgery in our hospital from April 2013 to January 2016 were randomly selected, who were confirmed by arthroscopy result as the standard. The patients were diagnosed by MRI, and the diagnosis results were compared with the results of simple clinical diagnosis by physician. Results Clinical diagnosis by physicians showed 48 patients with knee ACL rupture(partial fracture in 41 cases and total fracture in 7 cases), 12 cases of normal patients. The average accuracy of partial and total fracture was 79.78%, and the false negative rate was 23.5%. MRI showed 53 patients with knee ACL rupture(partial fracture in 45 cases and total fracture in 8 cases) and 7 cases of normal patients. The average accuracy of partial and total fracture was 88.56%, and the false negative rate was 13.7%. The diagnostic accuracy of MRI was higher than that of simple clinical diagnosis by physicians, and the difference was statistically significant(P<0.05). Conclusion MRI is more accurate than simple clinical diagnosis by physicians, but there were still possibility of missed diagnosis. Therefore, MRI should be combined with clinical comprehensive analysis by physician to improve the accuracy of diagnosis.
