摘要
目的探讨产后缺乳(气血虚弱证)患者采取健脾疏肝通乳方及低频脉冲电刺激结合治疗的临床效果。方法共计纳入96例产后缺乳(气血虚弱证)患者进行本次分组研究,分组方法为随机数字表法,根据患者治疗方法不同将其分为对照组与观察组,对照组48例给予低频脉冲电刺激治疗,观察组48例给予低频脉冲电刺激及健脾疏肝通乳方治疗,对比各组患者数据情况:临床疗效、治疗前后中医证候积分变化及雌二醇(estradiol,E_(2))及催乳素(prolactin,PRL)水平变化、乳房充盈程度、泌乳量及乳汁质量评分变化、母乳喂养情况、治疗不良反应、治疗满意率。结果与对照组患者的总有效率与治疗满意率相比,观察组患者各项数据均更高(P<0.05);观察组患者纯母乳喂养率高于对照组患者(P<0.05);观察组治疗出现1例口苦,对照组未见不良反应(P>0.05);对比各组患者治疗前的中医证候积分、E_(2)与PRL水平、乳房充盈程度、泌乳量与乳汁质量评分,差异无统计学意义(P>0.05),治疗后各组患者的各项指标均改善,治疗后观察组患者各项指标均比对照组患者改善更佳(P<0.05)。结论产后缺乳(气血虚弱证)患者采取健脾疏肝通乳方与低频脉冲电刺激结合治疗的效果较为显著,患者治疗后症状改善,可较好分泌乳汁,提升纯母乳喂养率,满足新生儿需求,且不良反应少,安全性较高,患者较为满意及认可。
Objective To investigate the clinical effect of Jianpi Shugan Tongru Prescription(健脾疏肝通乳方)combined with low frequency pulse electrical stimulation in patients with postpartum lactation deficiency(syndrome of weakness of Qi and blood).Methods A total of 96 patients with postpartum lactation deficiency(syndrome of weakness of Qi and blood)admitted from January 2022 to December 2023 were included in this grouping study.The grouping method was random number table meth-od,and they were divided into control group and observation group according to different treatment methods.The patients in the control group(48 cases)were given low-frequency pulse electrical stimulation treatment.Those in the observation group(48 ca-ses)were given low-frequency pulse electrical stimulation and Jianpi Shugan Tongru Prescription,and the data of patients in each group was compared,including the clinical efficacy,the changes in TCM symptom scores,the levels of estradiol(E_(2))and prolactin(PRL)before and after treatment,the changes in breast filling degree,lactation volume and milk quality score,breast-feeding status,treatment adverse reactions and treatment satisfaction rate.Results Compared with those in the control group,the total effective rate and satisfaction rate were higher in the observation group(P<0.05).The rate of exclusive breastfeeding in the observation group was higher than that in the control group(P<0.05).There was 1 case of bitter mouth in the observation group and no adverse reaction in the control group(P>0.05).The scores of TCM syndrome,the levels of E_(2) and PRL,breast filling degree,lactation volume and milk quality scores of both groups were not significant before treatment(P>0.05).After treatment,all indexes of both groups were improved,and all indexes of the observation group were better than those of the control group(P<0.05).Conclusion The combination treatment of Jianpi Shugan Tonglu Prescription and low-frequency pulse electrical stimulation for patients with postpartum lactation deficiency(syndrome of weakness of Qi and blood)has a more significant effect.After treatment,the symptoms of patients are improved.They can better secrete milk,improve the rate of exclusive breastfeeding,meet the needs of newborns and have fewer adverse reactions and higher safety,and the patients are more satisfied and recognized.
