摘要
乡村医生是农村安宁疗护模式的关键角色,其在参与安宁疗护服务时面临诸多挑战。通过对河南6个地级市35名乡村医生的半结构访谈质性研究显示,乡村医生的服务内容集中在身体照护层面,心理照护仅作为辅助手段,社会与灵性照护严重不足。同时面临阿片类药物使用权限受限、个人专业知识与技能不足、病情沟通中的文化障碍,以及福利与法律保障的双重缺失等现实困境。鉴于此,提升乡村医生的专业能力,并在农村广泛宣传安宁疗护理念,将有助于更好地推动安宁疗护事业在农村地区的深入发展。
Rural doctors constitute a pivotal actor within the rural hospice care model.Nevertheless,their participation in end-of-life services is beset by multiple constraints.A qualitative study,grounded in semi-structured interviews with thirty-five rural doctors from six areas in Henan Province,reveals that their services primarily focus on physical care,with psychological support provided only as an auxiliary measure,while social and spiritual care remain markedly under-developed.These doctors also confront a constellation of practical predicaments,including restricted access to opioid medications,insufficient professional knowledge and skills,cultural barriers in communicating about illness,and a dual absence of welfare and legal protections.To address these issues,enhancing rural doctors'professional competencies and promoting hospice care awareness in rural areas are essential steps towards strengthening hospice care in rural China.
