摘要
目的探讨低级别嗜酸细胞性肾肿瘤的临床表现、影像学特征及病理学特征。方法回顾性分析东阳市人民医院2015年1月至2024年9月经病理诊断确诊的6例低级别嗜酸细胞性肾肿瘤患者的临床资料、影像学表现、病理组织形态、免疫组织化学及随访资料。结果6例患者中男2例、女4例。6例患者肾脏内呈现为边界清晰的单发实质性肿块。4例为部分肾切除,2例由于肿物大且术前考虑恶性行肾脏根治性切除。显微镜下肿瘤边界清楚,肿瘤细胞排列成紧密的小巢状结构,细胞核呈圆形或卵圆形,胞质丰富,嗜酸性,核周有空晕。免疫表型中6例CK7均阳性,6例CD117均阴性,3/6例GATA3阳性。结论低级别嗜酸细胞性肾肿瘤影像学主要表现为肾脏实质性肿块。独特的病理组织学特征及免疫表型提示其是一种特殊类型的具有嗜酸细胞组织形态的肾脏实体肿瘤。低级别嗜酸细胞性肾肿瘤生物学行为呈惰性,治疗首选保留肾单位的部分肾切除。
Objective To explore the clinical manifestations,imaging characteristics,and pathological features of low-grade oncocytic renal tumors.Methods A retrospective analysis was conducted on clinical data,imaging findings,pathological morphology,immunohistochemistry,and follow-up information of six patients treated at Dongyang People's Hospital from January 2015 to September 2024 for pathologically confirmed low-grade oncocytic renal tumors.Results The six patients included two males and four females.Imaging studies revealed well-defined solitary solid masses within the kidneys in all cases.Four patients received partial nephrectomy,while two had radical nephrectomy due to large tumor size and preoperative suspicion of malignancy.Under the microscope,there were distinct tumor margins with tumor cells arranged in tight nests,featuring round or oval nuclei and abundant eosinophilic cytoplasm with a perinuclear halo.Immunophenotyping revealed that all six cases were CK7 positive and CD117 negative,and three of the six cases were GATA3 positive.Conclusions Low-grade oncocytic renal tumors primarily present as solid renal masses on imaging.These tumors exhibit unique pathological and immunophenotypic characteristics,suggesting that they are a distinct type of renal epithelial tumor with oncocytic morphology.They have an indolent biological behavior,and the treatment of choice is nephron-sparing partial nephrectomy.
