摘要
代谢综合征是一组以肥胖、高血糖、血脂异常及高血压病为特点的临床证候,其主要病理机制涉及胰岛素抵抗、代谢性炎症、脂质代谢、氧化应激、自噬等多系统紊乱。根据代谢综合征的全球流行病学调查,目前世界上有超过10亿人受到代谢综合征的影响。其不仅显著增加了2型糖尿病和心血管疾病患病风险,还与脂肪肝、多囊卵巢综合征等多种代谢性疾病密切相关。目前,主要采取分项干预策略,针对肥胖、血糖异常、血脂紊乱和高血压病等组分进行单独治疗,这种“分而治之”的治疗模式往往难以有效阻断疾病进展的共同通路,且存在药物相互作用和多重用药风险。参苓白术散具有健脾化湿功效,与代谢综合征脾虚痰湿的主要临床病机特征相契合。同时现有临床研究表明,其具有减轻体质量、降低血糖和血压、纠正脂代谢紊乱及延缓动脉粥样硬化的作用。文章系统地梳理和分析了近年来参苓白术散在改善代谢综合征的病理机制和疾病发展等方面的动态研究,为进一步探究参苓白术散在代谢综合征中的应用提供理论基础。未来或围绕代谢综合征的共同致病机制,研究参苓白术散能否实现代谢综合征“异病同治”的治疗目标。
Metabolic syndrome is a group of clinical syndromes characterized by obesity,hyperglycemia,dyslipidemia,and hypertension.Its primary pathological mechanisms involve insulin resistance,metabolic inflammation,lipid metabolism,oxidative stress,autophagy and other multi-system disorders.According to global epidemiological survey,over one billion people worldwide are currently suffering from metabolic syndrome.Metabolic syndrome not only significantly increases the risk of type 2 diabetes and cardiovascular diseases,but also is closely associated with various metabolic disorders such as fatty liver disease and polycystic ovary syndrome.Current modern medical practice primarily adopts a segment intervention strategy to treat obesity,abnormal blood glucose levels,dyslipidemia,and hypertension individually.This divide-and-conquer treatment model often struggles to effectively block the common pathways of disease progression and carries risks of drug interactions and polypharmacy.Shenling Baizhu San(参苓白术散)has the effect of strengthening the spleen and resolving dampness,which is consistent with the main clinical pathogenesis characteristics of spleen deficiency and dampness-phlegm of metabolic syndrome.Additionally,existing clinical studies have shown that Shenling Baizhu San has the effects of reducing body weight,decreasing blood glucose and blood pressure,restoring lipid metabolism balance,and delaying atherosclerosis.This article systematically analyzes recent research on the action mechanism of Shenling Baizhu San in ameliorating the pathological state and delay disease progression of metabolic syndrome,providing a theoretical basis for further exploring its application in metabolic syndrome.Future research may focus on the common pathogenic mechanisms of metabolic syndrome to investigate whether Shenling Baizhu San can achieve the therapeutic goal of“treating different diseases by the same method”for metabolic syndrome.
