摘要
研究表明,感音神经性听力损失(sensorineural hearing loss,SNHL)儿童在Romberg测试、单腿站立测试、布尼氏动作熟练度测试等平衡功能评估中,较健听儿童表现出更显著的不稳定性,且前庭功能异常发生率更高。影响SNHL儿童平衡功能的因素包括听力损失的侧别(单/双侧)、年龄及听力损失程度等。其机制可能涉及前庭功能障碍、听觉输入减少、认知负荷增加以及过度保护等多重因素。值得注意的是,助听设备(如助听器或人工耳蜗)佩戴可能对平衡功能产生积极影响。虽然目前相关研究主要集中于成人和老年人群,但初步证据表明,听觉输入的恢复可能有助于改善平衡功能。本文系统综述了SNHL儿童在静态平衡、动态平衡及前庭功能测试中的表现特征,深入探讨其影响因素及潜在机制,旨在为临床医师和康复治疗师提供理论依据,优化SNHL儿童的平衡功能与康复干预策略,促进其全面发展。
Research indicates that children with sensorineural hearing loss(SNHL)exhibit significantly greater instability in balance function assessments such as the Romberg test,single-leg stance test,and Bruininks-Oseretsky Test of Motor Proficiency compared to their normal hearing peers,along with a higher incidence of vestibular dysfunction.Factors influencing balance function in children with SNHL include laterality of hearing loss(unilateral/bilateral),age and the degree of hearing impairment.The underlying mechanisms may involve multiple factors such as vestibular dysfunction,reduced auditory input,increased cognitive load and overprotection.Notably,the use of hearing assistive devices(e.g.,hearing aids or cochlear implants)may have a positive impact on balance function.Although current research primarily focuses on adult and elderly populations,preliminary evidence suggests that restoration of auditory input could contribute to improved balance performance.This article systematically reviews the performance characteristics of static balance,dynamic balance and vestibular function tests in children with SNHL,as well as the influencing factors and potential mechanisms,to provide clinicians and rehabilitation therapists with a theoretical foundation for optimizing balance function and rehabilitation intervention strategies in these children,thereby promoting their holistic development.
