摘要
目的分析磁共振成像和多层螺旋电子计算机断层扫描(CT)在原发性肝癌患者诊断中的价值。方法选取2021年1月至2022年3月我院拟行诊治的疑似原发性肝癌患者50例,分别进行多层螺旋CT检查以及磁共振成像检查,以病理结果作为“金标准”,统计并比较多层螺旋CT检查以及磁共振成像的诊断结果、诊断效能,以及对于患者门静脉癌栓和肝内动静脉瘘的诊断准确性。结果50例疑似患者经“金标准”判定,确诊为原发性肝癌患者共计45例,病灶共计52枚,有肝内动静脉瘘患者16例,有门静脉癌栓者14例,多层螺旋CT检查确诊为原发性肝癌者共计38例;磁共振成像检查确诊为原发性肝癌者共计44例。磁共振成像对于原发性肝癌的诊断准确性、敏感度、特异度、阳性与阴性预测值均显著高于多层螺旋CT(P<0.05);多层螺旋CT、磁共振成像对于患者的门静脉癌栓以及肝内动静脉瘘的诊断准确性对比,差异无统计学意义(P>0.05)。结论在原发性肝癌的临床诊断中磁共振成像的诊断效能高于多层螺旋CT,更具临床应用和推广价值。
Objective To analyze the diagnostic value of magnetic resonance imaging and multi-slice spiral computed tomography(CT)in patients with primary liver cancer.Methods From January 2021 to March 2022,50 patients with suspected primary liver cancer in our hospital were selected,for multi-slice spiral CT examination and magnetic resonance imaging examination respectively.The pathological results were taken as the"gold standard"to statistically compare the diagnostic results and diagnostic efficacy of multi-slice spiral CT examination and magnetic resonance imaging.And the diagnostic accuracy of portal vein cancer embolus and intrahepatic arteriovenous fistula.Results Among the 50 suspected patients,a total of 45 cases were confirmed as primary liver cancer by the"gold standard".52 lesions were confirmed as primary liver cancer.16 cases had intra-hepatic arteriovenous fistula,14 cases had portal vein cancer embolus.And a total of 38 cases were confirmed as primary liver cancer by multi-slice spiral CT examination.44 patients were diagnosed with primary liver cancer by magnetic resonance imaging.The diagnostic accuracy,sensitivity,specificity,positive and negative predictive values of MRI for primary liver cancer were significantly higher than those of multislice spiral CT(P<0.05).There was no significant difference in diagnostic accuracy between multi-slice spiral CT and magnetic resonance imaging in patients with portal vein cancer thrombi and intrahepatic arteriovenous fistula(P>0.05).Conclusion In the clinical diagnosis of primary liver cancer,the diagnostic efficiency of magnetic resonance imaging is higher than that of multi-slice spiral CT,and it has more clinical application and promotion value.
