锥形束CT在种植病患者中的应用

摘要 目的分析对种植病患者开展锥形束计算机断层扫描成像(CT)的作用。方法选择在本院开展种植牙手术患者,共计90例,研究起止时间为2021年1月至2021年6月,收集全部病患临床资料,并实施回顾性分析。全部病患在进入医院后均开展口腔颌面曲面断层检查以及锥形束CT检查,以检查结果作为金标准。针对不同检查方式病患颏孔解剖检出情况进行对比;针对不同检查方式病患下颌部位垂直距离测量结果进行对比;针对不同检查方式种植手术后检查情况进行对比。结果锥形束CT检查在颏神经环、副颏孔、切牙管、颏棘上孔以及颏孔检出率方面均高于曲面断层检查(P<0.05)。在磨牙区,锥形束CT检查可用骨高度低于曲面断层检查,下颌骨根尖处根尖骨宽度低于曲面断层检查;在前磨牙区,锥形束CT检查下颌骨牙槽嵴顶跟间骨宽度低于曲面断层检查;在前牙区,锥形束CT检查骨类牙根长度低于曲面断层检查,可用骨高度低于曲面断层检查,下颌骨根尖处根尖骨宽度宽于曲面断层检查(P<0.05)。锥形束CT检查骨量不足检出率高于曲面断层检查,未发生骨结合检出率低于曲面断层检查(P<0.05)。结论种植病患者开展锥形束CT检查,能够在手术前了解颌骨解剖结构,有利于疾病诊断以及判断手术后种植体情况,具有一定的应用价值。 Objective To analyze the effect of cone-beam computed tomography(CT)in patients with implant disease.Methods A total of 90 patients were selected to undergo dental implantation surgery in our hospital,with a study period from January 2021 to June 2021.Clinical data of all patients were collected and retrospectively analyzed.After entering the hospital,all patients underwent oral and maxillofacial curved surface tomography and cone beam CT examination,with the examination results as the gold standard.Compare the anatomical detection of the mental foramen in patients with different examination methods;Compare the vertical distance measurement results of the mandibular region of patients with different examination methods.Compare the examination results after implantation surgery with different examination methods.Results The detection rates of mental nerve ring,accessory mental foramen,incisor canal,supraspinal foramen and mental foramen in cone-beam CT were higher than those in curved tomography(P<0.05).In the molar area,the available bone height of cone-beam CT was lower than that of curved tomography,and the apical bone width of mandible was lower than that of curved tomography.In the premolar region,the width of the intercalar bone in the alveolar ridge of the mandible was lower than that in the curved tomography.In the anterior region,the root length of bone teeth in CBCT examination was lower than that in curved tomography examination,the height of available bone was lower than that in curved tomography examination,and the apical bone width in the mandibular apical was wider than that in curved tomography examination(P<0.05).The detection rate of bone deficiency in cone-beam CT was higher than that in curved tomography,and the detection rate of no bone union was lower than that in curved tomography(P<0.05).Conclusions Cone-beam CT examination of patients with implant disease can understand the anatomical structure of the jaw before operation,which is conducive to the diagnosis of the disease and the evaluation of the condition of the implant after operation,and can play a certain application value.
