摘要
目的观察皮瓣修复治疗慢性难愈性创面的临床疗效。方法对2024年1月至2024年12月在新疆生产建设兵团第一师医院收治的20例慢性难愈性创面患者彻底清创后,给予皮瓣修复治疗。观察患者皮瓣修复术后皮瓣血运、切口渗出、局部红肿情况及创面愈合时间。结果所有患者术后皮瓣血运情况良好,切口渗出少,局部红肿等情况随着时间推移明显改善;术后4~7 d,20例患者皮瓣血运均趋于稳定,未再出现明显血运异常情况;术后第5天,12例患者切口仅可见少量浆液性渗出,8例患者切口已基本无渗出,敷料干燥;术后第7天,17例患者皮瓣局部红肿已明显减轻,皮肤颜色接近正常肤色,仅有轻微发红,3例患者红肿仍稍明显,但较前已有显著改善。创面愈合时间为11~29 d,平均(21.47±2.28)d。术后5~8 d,创缘上皮组织逐渐向中央覆盖,术后11 d左右创面大部分被上皮覆盖,剩余少许散在点状未愈合区域,继续换药处理后,术后25~29 d创面完全愈合。结论皮瓣修复治疗慢性难愈性创面可有效促进切口渗出及局部红肿消退,改善皮瓣血运,促进创面愈合。
Objective To observe the clinical efficacy offlap repair in the treatment of chronic refractory wounds.Methods From January to December 2024,20 patients admitted to the First Division Hospital of Xinjiang Production and Construction Corps receivedflap repair treatment after thorough debridement.The blood vascularity,incision exudation,local redness and swelling and wound healing time of the patient afterflap repair were observed.Results The postoperativeflap had good blood vascularity,less incision exudation,and local redness and swelling improved significantly with the passage of time.4-7 days after surgery,the blood vascularity of theflap in 20 patients tended to be stable,and there was no obvious abnormal blood vascularity.After the 5th postoperative day,only a small amount of serous exudate was seen in the incision of 12 patients,and there was basically no exudate from the incision in 8 patients,and the dressing was dry.By the 7th day after surgery,the local redness and swelling of theflap in 17 patients had been significantly reduced,and the skin color was close to the normal complexion,with only slight redness,and the redness and swelling in 3 patients were still slightly obvious,but there was a significant improvement compared with before.The wound healing time was 11-29 days,with an average of(21.47±2.28)days.After 5-8 days postoperatively,the epithelial tissue at the wound edge gradually covers towards the center.By around 11 days postoperatively,most of the wound is covered by epithelium,with a small amount remaining scattered in the unhealed area.After further dressing changes,the wound fully heals within 25-29 days postoperatively.Conclusions Skinflap repair for the treatment of chronic refractory wounds can effectively promote the resolution of incision exudation and local redness/swelling,improve skinflap blood supply,and accelerate wound healing.
