摘要
目的分析低剂量能谱CT联合自适应统计迭代重建技术(adaptive statistical iterative recon⁃struction,ASIR)在肺结节定性诊断中的应用价值。方法选择2021年1月至2023年12月在淄博市市级机关医院接受治疗的115例肺结节患者为研究对象。所有患者入院后均先接受CT检查,随后采用ASIR进行图像重建,并以穿刺病理活检结果作为金标准,对良恶性质实施定性鉴别。统计穿刺病理活检结果中良性结节数、恶性结节数占比。对比低剂量能谱CT联合ASIR与单纯CT检查的诊断效能值。结果穿刺病理活检结果中,阴性病例95例,占比为82.61%(95/115),阳性病例20例,占比为17.39%(20/115)。低剂量能谱CT联合ASIR检出阳性病例19例,单纯CT检查检出阳性病例28例。低剂量能谱CT联合ASIR的特异度、阳性预测值、准确度均高于单纯CT检查,差异均有统计学意义(P均<0.05)。结论肺低剂量能谱CT联合ASIR在肺结节定性诊断中展现出较高的临床价值,ASIR的应用对于提高肺结节诊断效率具有重要意义。
Objective To analyze the application value of low-dose spectral CT combined with adaptive statistical iterative reconstruction(ASIR)technology in the qualitative diagnosis of pulmonary nodules.Meth⁃ods A total of 115 patients with suspected pulmonary nodules who were treated at Zibo Municipal Government Agencies Hospital from January 2021 to December 2023 were selected as the research subjects.All patients un⁃derwent CT examination after admission,and then ASIR was used for image reconstruction.Needle biopsy re⁃sults were used as the gold standard to qualitatively identify benign and malignant lesions.The number of be⁃nign nodules and the proportion of malignant nodules in the biopsy results were counted.The diagnostic efficacy of low-dose spectral CT combined with ASIR and CT alone was compared.Results In the results of needle pathological biopsy,95 cases were negative,accounting for 82.61%(95/115),and 20 cases were positive,ac⁃counting for 17.39%(20/115).Nineteen positive cases were detected by low-dose spectral CT combined with ASIR,and 28 positive cases were detected by CT alone.The specificity,positive predictive value and accuracy of low-dose spectral CT combined with ASIR were higher than those of CT alone,and the differences were sta⁃tistically significant(all P<0.05).Conclusion Low-dose spectral CT of the lung combined with ASIR has dem⁃onstrated high clinical value in the qualitative diagnosis of pulmonary nodules.The application of ASIR is of great significance for improving the diagnostic efficiency of pulmonary nodules.
