摘要
目的:探究冠心病患者甲状腺激素水平与血脂异常的相关性,为冠心病的临床诊疗提供新的思路。方法:选取某医院心脏大血管外科冠状动脉粥样硬化性心脏病患者,对其血脂水平及甲状腺激素(TH)水平包括促甲状腺激素(TSH)、游离四碘甲状腺氨酸(FT4)、游离三碘甲状腺氨酸(FT3)水平进行数据统计。将所有数据进行归纳统计后进行单因素Logistic回归分析,随后将有差异项目代入相关性方程进行计算,最后分析甲状腺激素水平与血脂水平之间的关系。结果:共统计冠心病患者480例,其中符合纳入标准患者203例。通过对甲状腺激素行二元Logistic分析,发现FT4、年龄、高血压病史及吸烟史为脂代谢异常的危险因素。将FT4代入皮尔逊相关性方程分析后发现,游离四碘甲状腺氨酸水平与甘油三酯、高密度脂蛋白、低密度脂蛋白及脂蛋白间存在显著相关性。结论:FT4、年龄、高血压病史及吸烟史为脂代谢异常的危险因素,甲状腺激素(TH)水平与血脂水平之间存在显著相关性,而血脂水平是冠状动脉粥样硬化性心脏病的重要危险因素,因此,通过控制甲状腺激素水平,对冠心病患者的临床诊疗具有重要意义。
Objective:To explore the correlation between thyroid hormone levels and dyslipidemia in patients with coronary heart disease,and to provide new ideas for the clinical diagnosis and treatment of coronary heart disease.Methods:Patients with coronary atherosclerotic heart disease from the Department of Cardiac and Great Vascular Surgery of Anhui Medical University Affiliated Provincial Hospital from were selected.Data on their lipid levels and thyroid hormone(TH)levels,including thyroid stimulating hormone(TSH),free thyroxine(FT4),and free triiodothyronine(FT3),were collected and statistically analyzed.After summarizing all the data,univariate Logistic regression analysis was conducted.Subsequently,the items with significant differences were entered into the correlation equation for calculation.Finally,the relationship between thyroid hormone levels and lipid levels was analyzed.Results:A total of 480 patients with coronary heart disease were statistically analyzed,among whom 203 patients met the inclusion criteria.Through binary Logistic analysis of thyroid hormones,it was found that FT4,age,history of hypertension and smoking history were risk factors for abnormal lipid metabolism.After substituting FT4 into the Pearson correlation equation for analysis,it was found that there was a significant correlation between the level of free tetriodothyronine and triglycerides,high-density lipoprotein,low-density lipoprotein,and lipoproteins.Conclusion:FT4,age,history of hypertension and smoking history are risk factors for abnormal lipid metabolism.There is a correlation between thyroid hormone(TH)levels and blood lipid levels,and blood lipid levels are important risk factors for coronary atherosclerotic heart disease.Therefore,controlling thyroid hormone levels is of great significance for the clinical diagnosis and treatment of patients with coronary heart disease.
