摘要
目的:探讨Clark安适护理模式在改善晚期肿瘤患者症状管理、生活质量及心理健康方面的效果,为临床护理实践提供依据。方法:选取泰州市第三人民医院2022年1月—2024年9月收治的86例晚期肿瘤患者作为研究对象,随机分为两组,每组各43例。观察组予以Clark安适护理,对照组予以常规护理。比较两组干预前后症状管理指标[视觉模拟量表(VAS)、呕吐频率、疲劳(FACIT-F)]、生活质量(EORTC QLQ-C30)、心理健康指标[医院焦虑抑郁量表(HADS)、贝克抑郁量表(BDI)、心理压力量表(PSS)]的变化。结果:在干预结束后,观察组VAS评分、呕吐频率和FACIT-F疲劳评分均低于基线且低于对照组,差异有统计学意义(P<0.05)。观察组EORTC QLQ-C30功能性领域、总体健康评分高于基线且相较于对照组更高,症状域评分相较于基线及对照组均更低,差异有统计学意义(P<0.05);对照组干预前后心理健康各项指标比较,差异无统计学意义(P>0.05)。在干预结束后,观察组心理健康各项评分均低于基线,且评分低于对照组,差异有统计学意义(P<0.05)。结论:Clark安适护理模式在晚期肿瘤患者中应用,能够有效缓解症状,改善生活质量,减轻心理压力,提升整体护理效果。
Objective:To investigate the effect of the Clark Comfort Care model in improving symptom management,quality of life,and psychological health in patients with advanced cancer,thereby providing evidence for clinical nursing practice.Method:A total of 86 patients with advanced cancer admitted to The Third People's Hospital of Taizhou from January 2022 to September 2024 were selected as the research objects and randomly divided into two groups,with 43 cases in each group.The observation group received Clark Comfort Care,while the control group received routine care.Changes in symptom management indicators[Visual Analog Scale(VAS),frequency of vomiting,fatigue(Functional Assessment of Chronic Illness Therapy-Fatigue,FACIT-F)],quality of life(European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30,EORTC QLQ-C30),and psychological health indicators[Hospital Anxiety and Depression Scale,HADS;Beck Depression Inventory,BDI;Perceived Stress Scale,PSS]before and after the intervention were compared between the two groups.Result:After the intervention,the observation group exhibited significantly lower VAS scores,frequency of vomiting,and FACIT-F fatigue scores compared to baseline and the control group(P<0.05).In the observation group,scores in the functional domains and overall health and quality of life were significantly higher than baseline and the control group(P<0.05),while scores in the symptom domains were significantly lower than baseline and the control group(P<0.05).In contrast,the control group did not demonstrate significant changes in any indicators of psychological health(P>0.05).Post-intervention scores of psychological health in the observation group were significantly lower than baseline and significantly lower than those in the control group(P<0.05).Conclusion:The Clark Comfort Care model,when applied to patients with advanced cancer,effectively alleviates symptoms,improves quality of life,reduces psychological stress,and enhances overall nursing outcomes.Therefore,the Clark Comfort Care model is recommended for widespread clinical application to improve the quality of nursing care and life satisfaction in patients with advanced cancer.
