摘要
目的通过探讨基于功能对线(FA)理念切除参数的参考区间,为该技术标准化提供数据支撑。方法回顾性分析2023年1月至2024年1月解放军总医院第四医学中心骨科医学部接受机器人辅助FA-全膝关节置换术(TKA)的终末期膝骨关节炎患者71例的病例资料。术前利用Mako机器人系统构建三维模型规划手术。术中由机器人辅助完成操作。观察指标为经过手动调整后Mako系统上显示的切除参数。结果股骨远端截骨角度(varus为正)为0.6°(1.0°)[中位数(四分位距)]。胫骨近端截骨角度(varus为正)为1.0°(2.0°)。股骨假体低头角度(flexion为正)为3.5°(2.0°)。胫骨假体后倾角度为2.0°(3.0°)。股骨假体相对于经上髁轴线的旋转角度(rTEA)为(1.8°±1.9°);股骨假体相对于后髁轴线的旋转角度(rPCA)为(4.1°±2.0°)。结论研究FA的工作流程及切除参数可为FA技术标准化提供数据支撑。
Objective To explore the reference range of resection parameters based on the concept of functional alignment(FA),and to provide data support for the standardization of FA technique.Methods A single-center retrospective study was conducted,enrolling 71 patients with end-stage knee osteoarthritis underwent robot-assisted FA-TKA in Department of Orthopedics,the Fourth Medical Center of Chinese PLA General Hospital,from January 2023 to January 2024.Preoperative 3D modeling and surgical planning were performed using the Mako robotic system,with robot-assisted execution of intraoperative procedures.The primary outcome measures were the resection parameters displayed on the Mako system following manual adjustments.Results The distal femoral resection angle(positive values indicate varus)was 0.6°(1.0°)[M(IQR)].The proximal tibial resection angle(positive values indicate varus)was 1.0°(2.0°).The femoral component flexion angle(positive values indicate flexion)was 3.5°(2.0°).The tibial component posterior slope angle was 2.0°(3.0°).The rotation angle of the femoral prosthesis relative to the transepicondylar axis was 1.8°±1.9°,and the rotation angle of the femoral prosthesis relative to the posterior condylar axis was 4.1°±2.0°.Conclusion This study delineates the FA workflow and reports corresponding resection parameters could offer the foundational data to support the standardization of FA techniques.
