摘要
目的探讨家庭韧性在膀胱癌术后灌注化疗患者自我感受负担与生活质量间的中介作用,为采取针对性措施提供依据。方法2023年8月至2024年7月,采用便利抽样法选取太原市某三级甲等医院泌尿外科收治的膀胱癌术后灌注化疗的456例患者为研究对象,采用一般资料调查表、中文简化版家庭韧性评估量表(the shortened Chinese version of the family resilience assessment scale,FRAS-C)、自我感受负担量表(self-perceived burden scale,SPBS)、欧洲癌症研究和治疗组织生活质量核心问卷(European Organization for Research and Treatment of Cancer-quality of life questionnaire-Core 30,EORTC QLQ-C30)对其进行调查。结果膀胱癌术后灌注化疗患者自我感受负担与家庭韧性、生活质量的功能领域和整体健康水平呈负相关,与症状领域和症状单项条目呈正相关(均P<0.01);家庭韧性与生活质量的功能领域和整体健康水平呈正相关,与症状领域和症状单项条目呈负相关(均P<0.01)。家庭韧性在自我感受负担与生活质量的功能和整体健康水平、症状领域和症状单项条目间起部分中介效应,中介效应比为30.28%、27.11%。结论自我感受负担不仅能直接作用于膀胱癌术后灌注化疗患者的生活质量,还可通过家庭韧性的中介效应间接地影响其生活质量。
Objective To examine the mediating role of family resilience between self-perceived burden and quality of life in bladder cancer patients undergoing postoperative perfusion chemotherapy,providing evidence for targeted interventions.Methods A total of 456 patients undergoing postoperative perfusion chemotherapy for bladder cancer in the Department of Urology of a tertiary A hospital in Taiyuan City from August 2023 to July 2024 were selected by convenience sampling method,and they were investigated using the General Information Questionnaire,the shortened Chinese version of the Family Resilience Assessment Scale(FRAS-C),Self-Perceived Burden Scale(SPBS),and European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire-Core 30(EORTC QLQ-C30).Results Self-perceived burden was negatively correlated with family resilience,functional domains of life quality and global overall health status of quality of life,and positively correlated with the symptom domain and individual symptom items,while the family resilience was the opposite(all P<0.01).Family resilience played a partial mediating role between self-perceived burden and functional domains of life quality and overall health domain,and the symptom domain and individual symptom items.The mediation effect ratios were 30.28%and 27.11%,respectively.Family resilience partially mediated the relationships between self-perceived burden and overall health level(mediation ratio=30.28%)as well as symptom domains and single items(mediation ratio=27.11%).Conclusions The self-perceived burden of self can not only directly affect the quality of life of bladder cancer patients undergoing postoperative infusion chemotherapy for bladder cancer,but also indirectly affect their quality of life through the mediating effect of family resilience.
